Meet Tristan! He’ll be helping me around my physical therapy office this summer so you may see him at your check-in/out time or if you stop by in the mornings. Don’t worry, your appointment will still be just with me in the office.

Providing 1-1 care is one reason why I opened my own practice. I value being able to focus our sessions on YOU and YOUR needs (and not have 3-4 clients at a time like other clinics I’ve worked at).

5 years ago, during a time of so much uncertainty, an amazing team and I sent in this research to be published!

While we were working on it, I remember using Zoom for the first time- memorable because my kindergarten son brought a lizard into our cabin during a meeting and held it close to my face to see! I was happy the internet glitched at that moment, because I “may have” squealed in surprise!

After this was published, I had hoped to research and publish more. The time… is coming!

Treatment Options for Prolapse
Pelvic floor physical therapy and lifestyle changes are the first line treatment of prolapse! 

Pelvic floor physical therapy is non-invasive, has no known serious adverse effects, and is cost effective. Not all physical therapists are trained in the pelvic floor, so find one that is! 

Treatment is individualized based on your goals, examination findings, and symptoms.

Common treatments include:
* Learning how to correctly perform a pelvic floor contraction
* Developing strength, endurance, and coordination of the pelvic floor muscles with activity
* Pelvic floor dry needling to activate (or relax!) your pelvic floor muscles
* Bladder training and constipation management
* Fitting of a pessary, which is an intrava**nal device designed to provide support to the va**nal walls (I call it a sports bra for the pelvic floor)

Lifestyle Habits That Support Pelvic Health
* Drink at least 2 liters of fluids daily
* Increase your dietary fiber intake to 30-40g per day
* Proper toileting mechanics
* Don’t strain or push down during bowel movements
* Pre-contract pelvic floor muscles before sneezing, coughing, or doing high impact activities
* Stay active!!!

How is Prolapse Diagnosed?

A comprehensive medical and pelvic health history along with a pelvic examination assesses prolapse, which compartments are involved, and the staging.

The Pelvic Organ Prolapse Quantification (POP-Q) system is often used clinically to objectively characterize prolapse and provide a “staging” or grading system in relation to the location of the h***n (thin membrane or remnant of tissue usually ~1-2 cm of the va**nal opening).

Stage 0 indicates no prolapse
Stage 1 indicates the presence of prolapse above the h***n
Stage 2 prolapse is between 1cm above and 1 cm below the h***n
Stage 3 prolapse more than 1cm below the h***n, but less than 2cm from the end of the va**na
Stage 4 is a complete eversion of the va**nal vault

What is Prolapse?

Pelvic organ prolapse is the loss of support in the pelvic floor, resulting in the downward descent of the va**na, uterus, bladder, or re**um into or through the va**na.

This loss of support can be related to an underlying dysfunction in the pelvic floor muscles, connective tissue (like ligaments), or a combination of both.

Picture a hammock that has lost some of its tension. Your pelvic floor muscles and ligaments make up the “hammock” and when pressure is applied to the top of the hammock, the hammock lowers- along with everything on top of it. In the case of the pelvic floor, the structures on top of the “hammock” include the va**na, uterus, bladder, and or re**um.

Pelvic organ prolapse is often referred to as “POP.” A pr*****ed bladder (cystocele) is the most common type of pelvic organ prolapse. Men and women can experience rectocele (re**al prolapse).

Kegel’s aren’t the culprit… but they might not be the magic bullet treatment either.

“Kegel’s” are an activation of the pelvic floor. A closure of your va**nal and re**al openings, and a lift of your pelvic floor muscles… then a relaxation of these same muscles.

In other muscles, we (physical therapists) call this a “muscle set.”

It’s important to know how to activate (set) a muscle, but relaxation is also important.

Even more important, is coordinating the muscle contraction and relaxation with your normal activities. Make it functional for YOU!

What happens when it’s not functional?
