Did you know that goat meat is one of the healthier red meats? …. JK we aren’t eating this cutie. She and her momma are here to eat weeds and give us that that good 💩 for compost/fertilizer.

Ever heard of "goal oriented healthcare"? Its what happens when patients define their own health goals. Certainly most physicians could tell folks what their health goals "should" be, i.e. have a colonoscopy, get this biopsy, take your meds, lose weight, etc. But when we let patients define their own health goals, Shellinger et al showed that nearly half of them are non-medical goals! This is a reminder for all our healthcare heros, patients are more than a collection of organ systems so we need to address wellbeing in a holistic sense... mind, body, and spirit.

What is visceral fat and why is it so sinister? In contrast to subcutaneous fat, visceral fat resides closer or inside our internal organs. As noted in a BMJ study published last week, there is a strong case implicating visceral fat as a root cause of metabolic disease, inflammation, atherosclerosis, and even cancer. This is scary because visceral fat cannot be seen, is difficult to measure, and is sometimes present in normal weight individuals.

The take home message here is that we cannot rely on mirrors, scales, or BMI as sole measures of our wellbeing. Until we have an evidence based and widely available tool to measure visceral fat we have to include other surrogate tools to give us the complete picture. Examples of additional measures of health may include blood pressure, fasting blood sugars, cholesterol, waist to hip/height ratio, body fat scanners, or functional strength (e.g. push up, sit ups, pull ups).

With so much tech in healthcare, can wearable tech be used specifically for weight loss? Apple's Fitness+ is the latest in wearable fitness tech. Unfortunately, in a University of Pittsburgh study published in JAMA, addition of wearable fitness tech to a lifestyle program did NOT result in greater weight loss compared to the lifestyle program alone.

This doesn't necessarily mean not to go out and get the latest fitness tech if it aligns with your personal fitness goals. In fact, I regard tech as a great way to optimize an already successful fitness program. But it could be a mistake to think of wearable tech as a "quick fix" for your health or specifically for weight loss.

Whether you have the latest fitness tech or not, if the air is clean get out there and move, your mind-body-soul will thank you.

One of my favorite quotes:
"To me there are three things everyone should do every day. Number one is laugh. Number two is think -- spend some time time in thought. Number three, you should have your emotions move you to tears" -Jim Valvano

In Medicine there is a disproportionate focus on dying or not dying. We should focus at least equally on LIVING, and what it means to different people. As he had "tumors all over his body" the late coach Valvano gave us his take on what it means to Live... He said in his now famous speech to laugh, think, and cry each day.

reflect this weekend what it means to live, ask a friend, ask a patient, ask yourself.

Perspective is everything. One picture is a place we planned to vacation pre-pandemic, another picture is home. When I travel to beautiful places I inevitably wonder 2 things:
1) do locals here notice the beauty or take it for granted?
2) could someone from far away travel to my town and appreciate its beauty yet I'm too blind to see it?

What do you think? If you look at your neighborhood, town, city from a tourist perspective, would it change your view?

Have you heard of the Seven countries study? It was the work of Ancel Keys and the first international epidemiological study which analyzed the relationship between lifestyle factors, coronary heart disease, and stroke. Among hundreds of papers/books published, this one suggested low dietary fiber intake contributes to obesity even more than dietary fat intake. Studies have shown the average american eats less than half the RDA for fiber. Part of this may be our fixation with protein but rest assured, studies show the average american is eating well above the RDA for protein regardless of the trendy diet they choose. So to conclude, contrary to popular belief, the average american is likely at greater risk for a severe fiber deficiency, rather than a protein deficiency.

Ever had plain tofu right out of the box? I have, growing up this was a staple in our household. Didn't think much of it until I grew up and met people who found it strange to eat plain tofu. I couldn't believe it when I saw a recipe in this cookbook describing exactly how we ate tofu growing up... "fresh sliced tofu, add chopped scallion, shredded ginger, soy sauce, and enjoy" Does such a simple recipe even belong in a cookbook? Maybe it's all about perspective. Depending on how you look at it, plain uncooked tofu might be avoided due to its bland flavor; on the other hand, it might be celebrated for its delicate texture, mouthfeel, mild flavor, and its ability to accentuate the flavor of its garnishes such as scallion/ginger.
