Want Bigger but Natural – The Risks of Oversized Implants

During the consultation, the patient clearly expressed her goal: she wanted larger, fuller breasts, but still natural-looking.

Folded Breast Implant – Tight Pocket, Oversized Implant

During intraoperative assessment, the surgeon identified a clear issue: the implant was folding within the pocket.

Clinical findings:
- Multiple visible folds (2–3 distinct creases)
- Early signs of capsular contracture, compressing the implant
- Implant size (325cc) is relatively large for the existing pocket

👉 Root causes: Pocket too tight, not adequately dissected. Incomplete muscle release, limiting available space. Resulting in compression → folding → increased risk of long-term damage

👉 Technical insight: When an implant is placed in an insufficiently sized pocket, it cannot expand naturally. Continuous pressure from surrounding tissues leads to deformation, folding, and structural stress, which may eventually cause implant failure or rupture over time.

The patient presented after two previous breast surgeries with the following concerns:
– Breasts feel firm and rigid when lying down
– Persistent pain despite recent surgery
– Multiple hypertrophic scars, affecting aesthetics

👉 Surgeon’s assessment:
– The pain is mainly related to scar tissue and tension from the implants
– Previous implant sizes (420cc; 335cc) were relatively large, creating excessive pressure on breast tissue
– Scars tend to become thicker, raised, and more noticeable over time
