Folks, let's discuss about gamma knife radio-surgery for eye diseases.

Gamma knife was initially developed for brain surgery; it's application to eye tumors has been explored. Invented by Swedish neurosurgeon Lars Leksell in the late 1960s.

Known as GKRS /Gamma Knife Radio Surgery/ or GK-SRS /Gamma Knife Stereotactic Radio Surgery/ is a highly precise form of radiation therapy. Though it is called a surgery, Radio Surgery does not involve actual surgery & there is no knife used.

It is a non-surgical and an efficacious primary mode of treatment using radiation therapy to treat some eye conditions like uveal melanoma & choroidal metastasis.
Highly focused beams of Gamma Radiation is directed precisely to the target tissue (tumor); minimizing damage to the adjacent healthy tissues.
This technique is more useful in patients who are systemically unwell for a general anaesthesia.
Generally, only one treatment session is needed for a lesion. Treatment time depends on the type & location of the area being treated & it could last from a few minutes to a few hours. Performed as a one-day , out patient procedure.

Indications are-
👉Large intra ocular melanomas.
👉Choroidal melanomas withextra scleral extensions.
👉Choroidal melanomas in close proximity to Optic nerve.

It has more advantages over the conventional surgeries due to its precise targeting, limited side-effects, local tumor growth control & its non-surgical nature.

Studies suggest that increased contact with blue 🔵& green🟢 spaces can improve visual acuity & may help improve myopia.

The 'Blue Space Effect' refers to the positive impacts on human mental 🧠 & physical health 💪from exposure to and interactions with water bodies, including ocean, rivers, lakes & the aquatic environments. 🌳🌊🏞.

Spaces like nature and man-made water features can have a positive impact on visual acuity 👁 levels in youth.

The exact mechanism is not known. Some likely explanations for the link between blue🔵/green🟢 spaces & vision 👀 are :-
👁The calming effect of blue/green spaces may reduce eye strain.
👁Increased exposure to natural light can have a beneficial effect on eye health .
👁Increased relaxation and focus, indirectly helps eye health.

Shingles or Herpes zoster is caused by the virus, Varicella-zoster.

Findings from a recently concluded; Zoster Eye Disease Study- a multinational, randomized clinical trial presented at AAO 2024, showed that patients who have shingles in the eye & keratitis or iritis from HZO benefit significantly from 1000 mgs of daily Valacyclovir for a year.

The study also found that, when the treatment ended at 12 months; there was no statistically significant decrease in keratitis or iritis flare-ups. However; it was changed at the 18 month follow-up. Those treated with Valacyclovir showed a 27% lower likelihood of new or worsening disease.

Usually people who suffer multiple flare-ups can go on to develop chronic & recurrent disease & loss of vision.

They also found that treatment with lower dose Valacyclovir reduced the need for neuropathic pain medications.

Falling under the class of medicines called as semifluorinated alkanes. It works by forming a film layer on the eye to prevent tears and moisture from evaporating thereby the signs and symptoms of dry eye disease is reduced drastically.
Contact lens users should wear it only after 30 minutes of its usage. It can be used four times a day. It was approved by the FDA in the year 2023.

*Scratching remains an irresistible behavior in humans and animals [ including Fish ] suggesting an involutionary purpose preserved over millions of years.

Regardless of its significance in ophthalmology; yes, why do we scratch ?? A recent study on mice seems to be telling us the reason for it. The study published in the journal 'Science' revealed that *scratching regulates the immune response and composition of the skin microbiome, potentially evolving as a defense against skin infections.

Though *Scratching in response to itching is pleasurable, it is also harmful 😆.

In a major breakthrough in regenerative medicine, scientists have restored vision in patients using stem cells.

It offers new hope to millions.

A clinical trial in Japan in 2022 has successfully restored vision in multiple patients using a radical stem cell transplant. Researchers at Osaka University Hospital used induced pluripotent stem cells derived from healthy human blood cells to generate corneal epithelial cell sheets.

These sheets were transplanted onto the damaged corneas of four patients suffering from limbal stem cell deficiency (LSCD), a condition that causes scar tissue buildup and vision loss.

Two years after the procedure, the results show significant improvement, with three patients experiencing clearer vision and more transparent corneas. Unlike traditional transplants, this approach eliminates the need for donor corneas, offering hope to millions suffering from cornea-related blindness.

While the procedure remains experimental, it marks a major milestone in regenerative medicine.

Previous attempts at using stem cells focused on macular degeneration, but this is the first successful application for corneal restoration. Despite promising results, one patient experienced a decline in vision after a year, possibly due to an immune response. Researchers are now planning larger clinical trials to refine the technique and improve its long-term effectiveness.

If successful, this innovative treatment could revolutionize vision restoration, providing a sustainable and scalable alternative to corneal transplants for those in need.

Latest medical facts on drug related adverse effects indicate a potential side effect of the GLP-1 ( Glucagon-like peptide 1) receptor agonist drugs.

People with type-2 diabetes mellitus taking GLP-1 medicines may experience a potentially blinding eye condition as soon as the day after starting these drugs.
People are using them for other indications too. Microdosing is also done for various reasons like; weight maintenance, metabolic health improvements, anti-inflammation, & anti-autoimmune effects.

The studies have found a potential link between semaglutide and NAION ( Non-Arteritic Ischaemic Optic Neuropathy), papillitis, paracentral acute middle maculopathy.

NAION is actually a stroke of the optic nerve - the nerves that connects eye to the brain and potentially causing irreversible vision loss in one or both eyes.

It is still unclear as to what may cause a person taking GLP-1 medication to experience an eye issue. They assume that rapid changes in blood glucose levels might contribute to these eye complications. When the blood glucose is rapidly reduced; it can cause swelling in the optical nerve due to an osmotic shift.

So, it is recommended that if you experience a change in your vision while using any one of these class of drugs, you should cosult the prescribing doctor and see an ophthalmologist.

A 'cold stimulus headache', otherwise known as a *brain freeze or as an *ice cream headache is usually a rare form of brief pain or headache commonly associated with consumption of cold beverages or foods like ice cream etc.

It is not serious or harmful and goes away in a few seconds or minutes.

When a cold substance suddenly touches the roof of the mouth, the nerves in that area is stimulated and causes rapid constriction and swelling of blood vessels ; referring pain from roof of the mouth to the head. The quick change in the blood vessel size causes sudden pain. It is the brain's natural way of protecting the body.

If you get one, try to get the temperature in mouth and throat back to normal and the headache should subside. 😆
