!!4TH DOSE FOR IMMUNOCOMPROMISED ADULTS!!

We can now cater you from 9am-3pm, Mondays thru Fridays, at all VACCINATION SITES!

WHO ARE THE IMMUNOCOMPROMISED ADULTS? These are the individuals with challenged immune systems secondary to guarded medical conditions, as follows:
*Persons with an immunodeficiency
*People living with HIV
*Cancer patients (active)
*Transplant recipents
*Patients taking immunosuppresive drugs (steriods, etc.)
*Patients with poor progonosis or bed-ridden patients

Eligible individuals can get their 4TH DOSE at least three months from the 3RD DOSE.

For FULLY VACCINATED INDIVIDUALS, you may now request for your Mandaue City Vaccination Card with QR Code. Just go to http://tinyurl.com/MandaueQRVax

You will be notified through text once your Vaccination Card is ready for pick-up. You can claim your new Vaccination Card at the Mandaue City Sports Complex.

Please secure an Affidavit of Loss if you lost your vaccination card.
