I have now been pregnant for 18 of the 24 months of this pandemic. My babies and I have almost made it through all of the waves and emotions of these past two years.

As a pregnant HCW, I owe my family to the vaccine. I’m so thankful to be vaccinated, boosted, and reboosted.
šŸ’‰I received my first shot when I was four days postpartum with my girl, and transferred antibodies through my breast milk to protect her.
šŸ’‰I was boosted and reboosted in the first and third trimesters with my little guy, passing protection onto him and reducing his risk of Covid hospitalization after birth
šŸ’‰ Through being vaccinated I’ve reduced his risk of stillbirth, growth restriction, and preterm delivery
šŸ’‰ I’ve reduced my risk of severe illness and even death

In March 2020, I would never have been able to predict that this is how the next two years were going to unfold for me. But I’m so grateful that science has protected me through it.

Behind every great surgeon is a great surgical assist. šŸ”Ŗ

This lady has assisted me on almost every single case since I started my practice. She goes out of her way everyday to make sure that the patients at are well taken care of in the operating room. I don’t know what and I would do without her. And most importantly, she’s a wonderful person, mother, and friend.

Breastfeeding and working has been one of the biggest challenges I’ve faced as a new mom. For me, it really has taken a village.

The logistics:
- The amount of gear required to lug around the hospital and office
- Carving out time on your busy days to pump
- Rock hard, painful breasts in the operating room because you’re in a long case

The feelings of inadequacy despite best efforts:
- Decreased supply because I’m not pumping enough
- Pressure to make up extra pumping sessions instead of sleeping late at night or early in the morning
- The need for baby to start formula because it’s just not enough milk

But I’ve had an amazing support system in place to help me work through it. The first picture is my fellow breastfeeding mama, Abigail Pothen WHNP, who is my work breastie and amazing colleague. The second picture is of Aymber, one of my medical assistants who graciously donated her breast milk to me after hearing my anguish over formula refusal. And not pictured, but ever important, is my husband who washes all the parts, 24/7.

I may not make it as long as other moms, but I’m trying, and I’m so thankful to the people who have gotten me this far. If you see a breastfeeding mom at your workplace: open the door when you see her carrying 3 different bags, bring her a glass of water, or don’t sound annoyed when she tells you she has to go pump. We’re all just doing our best (.)(.) šŸ„›

In many cases, physical therapy is a successful option for treating these issues. However, sometimes women may need a bit more! Pessaries are inserted into the vaginal canal to offer additional support to the surrounding tissues. These devices can often offer a huge amount of relief and allow women to perform their day to day activities, including exercise, with minimal symptoms! ⁠

Pessaries are typically prescribed and fitted by a Urgogynecologist, who is a MD that specializes in female pelvic medicine! But guess what!? These amazing doctors will usually prescribe these devices IN CONJUNCTION with physical therapy!⁠

Enhanced recovery after surgery Part 3: outpatient management⁣

This is on my first outdoor walk with babyšŸ‚ā˜€ļøon post-op day 3! These are the ERAS principles I applied to my outpatient recovery:⁣
⁣
āœ”ļøDecreased narcotics intake: When I was discharged, for one week I continued to take Tylenol and Ibuprofen around the clock, whether I had pain or not. By doing this, I was ā€œstaying ahead of the painā€ and proactive so that I didn’t wait for the pain to creep up and become severe before doing something about it. I still needed a few narcotics pills during the first two days at home, but overall was feeling relatively back to normal 6 days after surgery.⁣
⁣
āœ”ļøBowel regimen: I continued with my daily Colace, Miralax, Probiotics, and Fiber daily with the addition of Milk of Magnesia when I needed a little extra help going. I noticed that if I was having regular bowel movements, my pain was so much better!⁣
⁣
āœ”ļøAmbulation: My body was so sore, but I kept moving! This was really the hardest part of my recovery because it was difficult to even get in and out of bed the first few days after the CS. But I really pushed myself to take small walks each day and found that each day things became easier and easier. Walking also helps prevent blood clots!⁣
⁣
āœ”ļøUsing support: This is also a huge part of ERAS and any recovery process. Use the support around you! I had my husband and parents there to help me so I could rest and sleep.

Enhanced recovery after surgery Part 2: Inpatient management⁣

During the inpatient phase of my C Section, I used the following principles of ERAS. ⁣
⁣
āœ”ļø Goal directed fluid therapy: Which means trying to keep your net fluid balance as close to 0 as possible šŸ’‰ ⁣
⁣
āœ”ļø Decreased narcotic intake: Through the use of spinal anesthesia during the CS. Post-op I used scheduled Toradol, which is an IV form of Ibuprofen, and scheduled oral Tylenol in the first 24 hours after surgery. After that, my pain was controlled with just oral Ibuprofen and Tylenol around the clock. However, I learned that CS recovery pain is no joke, and I definitely did need a few narcotic pills here and there for pain, but did not need them regularly. Ā šŸ’Š ⁣
⁣
āœ”ļø Early removal of catheter: to encourage me to move move move and reduce my risk of UTI šŸ’¦ ⁣
⁣
āœ”ļø Early ambulation: Although it wasn't easy, I knew that the sooner I moved, the better my pain control would be and the sooner my bowels would start working. Therefore I was up and moving around as soon as I had feeling back in my legs from the anesthesia, this was about 6 hours after surgery. I made sure that every few hours I was at least walking around my room, and even took my baby for a stroll in the hallways less than 24 hours after my surgery! šŸš¶šŸ½ā€ā™€ļøā£
⁣
āœ”ļø Early return to regular diet: I luckily did not experience much nausea, therefore was hungry and ready for my deli sandwich a few hours after surgery 🄪 ⁣
⁣
āœ”ļø Bowel regimen- I made sure to continue my bowel regimen because the less bloated and constipated you are, the less pain you’ll have šŸ’© ⁣
⁣
Recovering from a CS is recovering from major abdominal surgery and is not easy. If you're not able to do all of the above, that's OK, everyone's recovery is different! However, applying as many components of the ERAS pathway as you can, will help you get back to your baseline faster. ⁣
⁣

I recorded this video over the summer and it could not ring more true. Being in the 4th trimester is TOUGH! Some days I don’t even feel like the same person I was before. And I’m learning that I’m not the same person as before! Although I have so much love for my new little human, I’ve never been this tired or so full of emotions.

I’ve learned to lean on so many people and am so lucky to have my amazing husband, support system, and OB. I’ve also learned that it’s ok to struggle, yell, cry, and be happy all at the same time.
