Social determinants of health have a significant impact on a person’s well-being and quality of life, and they play a crucial role in a person’s overall health, particularly as they age. If left unresolved, these social barriers to health (i.e., lack of transportation, access to food, financial insecurity, gaps in education, and inadequate support systems) can lead to negative patient outcomes.

Like many Americans, Dennis Eleuteri is living with several chronic conditions that affect his day-to-day living. Care isn’t always financially accessible – especially for patients like Wanda, who are living with a chronic condition and facing high prescription costs.

When Wanda struggled to afford her medication, she called Monogram Health FIRST and a Monogram Health social worker collaborated with a Monogram Health pharmacist to find an alternative medication for her condition as well as a coupon to reduce the price of that new medication.

Monogram’s model is designed to meet people where they are – and in this case, it’s ensuring Wanda has access to the COPD medication she needed. Improving health outcomes starts with understanding the real challenges patients face every day.

Monogram Health is thrilled to announce our partnership with Aetna (a CVS Health Company) has expanded to Florida. Now, all eligible Aetna Medicare Advantage members across the state, who meet the program’s criteria, will benefit from Monogram’s value-based, in-home, multispecialty care for patients suffering from polychronic disease.

Like many states, Florida faces a shortage of healthcare professionals, rising healthcare costs, and the need to expand interventional services. Monogram Health has a proven track record for driving positive clinical outcomes, decreasing hospital and ER admission rates, and increasing star ratings, resulting in reduced medical costs and a high quality of living for all engaged patients.

To date, the CDC reports that kidney disease, heart disease, diabetes, and lower respiratory disease are among the top 10 leading causes of death in Florida. Along with Monogram Health’s patient-centric, multispecialty treatment plans, eligible, Florida-based Aetna patients (nearly 13,000) will have 24/7 access to personalized care from doctors, nurse practitioners, specialists, pharmacists, registered nurses, and social workers all in one place. Keeping physically active can help offset many effects of aging. Exercise helps improve mobility, reduce feelings of anxiety and depression, and contribute to better brain function. For those with chronic conditions, the CDC recommends at least two and a half hours of aerobic activity each week and two days of muscle-strengthening activity. If your mobility is limited, talk with your provider about movements you can do within your comfort zone.
