𝟐𝟒 𝐡𝐨𝐮𝐫𝐬 𝐨𝐧-𝐝𝐮𝐭𝐲

Alinsunod sa isa sa mga programa ni 𝐊𝐚𝐩𝐢𝐭𝐚𝐧 𝐔𝐥𝐲𝐬𝐞𝐬 "𝐓𝐞𝐭𝐞𝐭" 𝐌. 𝐑𝐞𝐥𝐥𝐚𝐦𝐚 kasama ng kanyang mga Konsehal, ang duty ng Barangay Health Workers (BHW) simula noong Nobyembre 16, 2023 ay “Bente Quatro Oras”.

Ito ay upang magbigay ng agarang aksyon sa ating mga kabarangay sa oras ng pangangailangang medikal (gamot, first aid).

All interested couples who is permanently residing in the City of Sto. Tomas are very much welcome.

3. Signature of the Parent/s if either or both of the couple/s is minor (18-24). "Personal appearance of the parents and ID
are required. Death certificate if either or both parent/s are deceased.
4. Voter's certification / ID with picture.
5. Certificate of Residency issued by the Barangay where the couples resides.
6. Death Certificate of the deceased husband or wife for those who have previous marriage.
7. CEDULA

Free: CENOMAR, Application for Marriage License and Solemnization Fee.
