These photos are 7️⃣months apart.
I saw this wonderful lady for 5ml of dermal filler over 2️⃣sessions. It is a good example to see how undereyes can look even after 7️⃣ months of treatment. I used (my favourite) Teosyal Redensity 2 in the under eyes.
In her last session, we used 1ml of lip filler with some work around her smile lines.
This is actually not the perfect candidate for tear trough filler and surgery is a good alternative. However, sometimes there are other factors we need to consider in our consultation. We are both very happy with what we achieved so far.
(One thing I want to criticise - the pictures are not taken from the exact same angle. I am trying to be more consistent with this!😬)

Bruising is sometimes inevitable. It’s a little bit frustrating when you bruise but the good news is it resolves in a few days and doesn’t affect the outcome of your treatment.

Virgin lips that already had great shape. This lovely girl had the most beautiful smile 😍, the only thing that bothered her was the “thinness” of her lips when she smiles.
I used 0.6ml of Belotero (one of my absolute favourite products 🤩).
The trick 💡 is to build it up slowly and ALWAYS working with your natural anatomy. Adding a lot of product to the corners of the lips can give rise to the dreaded ledge/shelf/🦆.
The beauty ✨ of injectables is not on how BIG you can go, it’s making sure that whatever we enhance suits your face in a balanced way.

This was something my patient has been thinking about for a while. By placing dermal filler 💉carefully, we can replace some of the lost volume in this area and reduce the hollowing. It’s a trick of the light 💡 that makes her under eye look brighter and fresher. What do you think?

Are you worried about starting a retinoid cream? If you’re thinking about incorporating retinoids to your regime or you haven’t used one in a long time, you may want to ease into them. This reduces some of the known side effects of using retinoids like the redness and peeling that a lot of people are worried about.😰
📝I have listed some over the counter products you may wish to try. They are not all the same and have different active ingredients (check out my previous posts on retinoid activity for more). You can start with the weaker forms and slowly build up. For example, use a pea sized amount every other day for a 4 weeks then slowly increase to every day.
☀️ Retinoids can make the skin more sensitive to UV radiation so sun protection is particularly important when using retinoids. You don’t need to use multiple retinoids (one is enough)!

Fear of lumpy lips is one of the biggest concerns for people wanting dermal fillers 😰. Lips CAN look and feel a little uneven after having dermal filler treatment. This is because of post procedure swelling, which usually subsides after 2-4 weeks. Lumps however are a separate issue that can occur for a number of reasons:
1️⃣ 𝐃𝐞𝐫𝐦𝐚𝐥 𝐟𝐢𝐥𝐥𝐞𝐫 𝐭𝐲𝐩𝐞: There are certain products that are designed for soft and mobile areas such as the lips. Placing an unsuitable product in the lips can make them look stiff and lumpy. Lumps can also occur with products that integrate poorly, migrate, or attract too much water.
Please remember that HYALURONIC ACID based dermal fillers are safer and are less likely to give rise to problems. Permanent or semi/permanent fillers are not suitable.
2️⃣𝗜𝗻𝗷𝗲𝗰𝘁𝗶𝗼𝗻 𝘁𝗲𝗰𝗵𝗻𝗶𝗾𝘂𝗲:
Although lumps can happen to anyone, larger more noticeable lumps are more likely to happen with less experienced practitioners. Amount of product, needle diameter, technique, depth and angle of injection are all factors that can contribute to the risk of getting a lump.
When a small superficial lump is present, early massage can help soften the appearance.
If there is too much product or if the product has been placed incorrectly, dissolving may be needed.
3️⃣𝗜𝗻𝗳𝗲𝗰𝘁𝗶𝗼𝗻
This presents as hot, swollen, tender bumps and can be acute in onset. Aseptic technique in a clean clinical environment is crucial to reducing the risk infection.
4️⃣ 𝗥𝗲𝗮𝗰𝘁𝗶𝘃𝗲/𝗵𝘆𝗽𝗲𝗿𝘀𝗲𝗻𝘀𝗶𝘁𝗶𝘃𝗶𝘁𝘆
An allergic reaction can develop at the site of filler placement. The reaction can be immediate or can occur after several weeks. Lumps can also happen in times of high immune activity e.g. flare up of an autoimmune disease or a viral infection such as flu.
The appearance of lumps on the lips seems to be more common than in other areas, possibly due the thin mucosa and their mobile nature.
𝗦𝗼 𝗵𝗼𝘄 𝗰𝗮𝗻 𝗶𝘁 𝗯𝗲 𝘁𝗿𝗲𝗮𝘁𝗲𝗱?
Your practitioner should ALWAYS provide you with a review to assess your lips. There are many possible treatments for lumpy lips and often they are responsive to dissolving the filler. @ London, United Kingdom

The eyebrow/eyelid droop might be something you have heard. They can be confused for one another by practitioners and patients alike.
EYELID droop is quite rare and happens when the eyelid muscle is affected by the neurotoxin (aka Botox)
EYEBROW droop is more common and happens when the forehead muscle is overtreated. You may feel like your “eyes are tired” “heavy” or “find it difficult to apply mascara/eyeliner”.
Fortunately, the effects of both are temporary and with time everything should return back to normal. It is important to evaluate the cause of the droop, your provider may need to adjust the dose/technique to avoid the problem in the future.

Injectables are often used for treating gummy smiles. However, there are times when this just isn’t a suitable treatment option. Injectables will only work when the cause of the gummy smile is overactivity of the lip elevator muscles - although in some cases such as, they can be used to camouflage. If there is another underlying cause (e.g. dental/bone structure) alternative treatments may be a better option so getting the diagnosis correct is KEY

My patient’s concern was what she described as a “weak chin”. This is something she has noticed in her profile for a long time. Dermal fillers can be an option for people who don’t want to undergo more invasive procedures. By lengthening her lower face we are able to create a more balanced profile.

Chin augmentation procedures can often take a lot of product because it is important that we blend the projection of the chin with rest of the lower face, making it look as natural as possible.

In this case, I used 5ml of dermal filler. ⚠️ It is SO important that you discuss expectations, budget and alternative options with your practitioner before.

This mountain is a (simplified) analogy of the ‘journey’ different retinoids must go through in order to reach the active state (Retinoic acid). The shorter the journey, the stronger the retinoid (i.e. the less steps in the conversion pathway, the more readily the retinoid converts to active retinoic acid).

The conversion pathway is as follows:

Retinol esters > Retinol > Retinaldehyde > Retinoid acid

In my previous post I explained the different types of retinoids available.

1. Prescription retinoids have the most evidence behind them particularly for ageing/acne skin.

3. A few studies have shown that retinaldehyde can be useful as topical a cream particularly for aged skin (with less skin irritation).

4. Studies on retinyl esters like (retinyl acetate and retinyl palmitate) have shown very limited results in ageing skin with little significant improvement. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2699641/

5. Hydroxypinacolone Retinoate (HPR) is new to the game with very limited research on its effect on photo ageing/acne. It was made by popular by The Ordinary and is commercially known as “Granactive Retinoid”

What’s retinol? What’s retinoid? They all sound the same, what’s the difference? It can all get so confusing! I’m going to try and break it down into a few posts over the coming week.

1️⃣Retinoids’ is the ‘ umbrella’ under which everything falls under. Retinoids are derived from vitamin A. They have found their way into our skin care for the last 30 years. There is also some research being done on their use in skin cancers.
2️⃣The further up the pyramid one goes the stronger the retinoid which goes hand in hand with the level of potential irritation it can cause your skin. Bare in mind using the strongest retinoid doesn’t necessarily mean it’s the best for you.
3️⃣Over the counter retinoids have less scientific evidence compared to prescription retinoids.

There are some side effects with retinoids such as dryness, peeling, redness and tightness. That’s completely normal and it will subside. ⚠️It is really important that you use SPF when using retinoids. ⚠️They are not to be used if you are pregnant or breastfeeding.

I recommend speaking to a dermatologist/professional if you are unsure.

Scrubs have had a lot of bad press in recent years. There is a belief that harsh ingredients in scrubs can result in ‘micro-tears’ in the skin. But are scrubs really harmful?

🧴Scrubs are physical exfoliators. The idea is that they remove the top layer of the skin known as the ‘stratum corneum’ and allow for new brighter looking skin to appear.

🧪What are they made of?
The most abrasive particles are made from fruit pits or aluminium oxide. These can be particularly abrasive and harsh on the skin due to their irregular shape.
Scrubs including sodium tetraborate decahydrate are less abrasive because they have smaller, regular shaped particles and dissolve as you apply them.
Some scrubs contain polyethylene (plastic microspheres) which aren’t necessarily damaging to the skin, but they are environmental pollutants.

📃The verdict:
There aren’t specific studies investigating the extent of micro tears from scrubs.
Generally, people who have sensitive skin should use them with caution and avoid scrubs containing harsher abrasives such as fruit pits or volcanic ash.

⚠️Be sensible and be gentle with your skin. Avoid rubbing too hard and for too long. Instead try to rub gently in small circular motions. Over-exfoliation can result in damage to the stratum corneum, resulting in dry skin, redness and irritation as well as exacerbating pre-existing acne.
