New topic and questions. We see compression neuropathies quite often. What are sites of nerve compression in the Upper Extremity of the median nerve, ulnar nerve, Radial nerve, musculocutaneous nerve (lateral antibrachial cutaneous nerve, Axillary nerve, and suprascapular nerve? A lot of nerves for us to discuss. Let the fun begin! GBS

I had a patient with rheumatoid arthritis who was unable to actively extend the MP joints of the hand. No known injury. When I passively place the MP joint in extension, the patient cannot actively maintain the extensor position. A tenodesis is effect is present ( with Wrist dorsiflexion, the fingers flex and wrist volar flexion, the fingers extend). What is your differential diagnosis of inability to extend the MP joints and what is your final diagnosis? Dr GBS

I have a patient who I performed an open carpal tunnel release on. Volar to the nerve within the carpal tunnel, I saw a large blood vessel. I have 3 questions regarding this: What blood vessel do you think it was? Where does this blood vessel come from? What anatomical variation in the nerve in the carpal tunnel is associated with this finding? Gary schwartz md (GBS)

Some of my patients with carpal tunnel syndrome tell me that their symptoms are worse at nighttime. I tell them that this has nothing to do with the sun going down. So, why do the patients have more symptoms when the sun goes down at nighttime? GBS

A patient sustained a complete laceration to the extensor digitorum communis tendon of the ring finger in zone 6. When you ask the patient to actively extend the ring finger, he/she is able to do this, although without full strength. How is the patient able to extend the digit with a laceration to the EDC tendon at that level? GBS

I have a patient who underwent an amputation at the DIP joint level of her index finger. She has gone through 5 months of hand therapy. Whenever she tries to flex the index finger, it goes into an extended position. Why does this occur and what is this phenomenon called? GBS

I have 2 cases that I initially put on another page when I did not how to work this Facebook thing, but Ylisa Young came to my rescue .

1) If I want to identify the superficial radial Nerve in the forearm, what interval do I have to go they (between which tendons). If the nerve is compressed at that level, what is the syndrome called?

2) I have a patient with a Posterior interosssous Nerve palsy. When I ask him/her to dorsiflex the wrist, it goes into a radial deviation posture. What is he anatomic reason for this to occur?
