VAGINAL THRUSH:

Risk Factors:
- Diabetes Mellitus
- Immunosuppression
- Pregnancy
-Insertion of IUD
- Systemic antibiotics

Signs and symptoms:
-itching of vaginal and vulval areas
-vulval redness, swelling or soreness
-painful urination
-vaginal discharge (white and creamy but odourless)

Treatment: Fluconazole 150mg is an oral medication to treat vaginal thrush. Alternatively, you can use topical clotrimazole preparation (1 day, 3 days or 6 days) to treat vaginal thrush.
*Pregnant ladies are suggested to use topical preparation instead of oral medications.

What to expect?
Symptoms should improve within 24 hours, however, complete symptoms relief happens in 2 days. Patient can choose to take oral fluconazole + topical clotrimazole cream for faster symptoms relief.

Adverse effects:
Generally well tolerated with incidence of > 1% experiencing nausea, vomiting, abdominal pain, headache and dizziness.

Referral points:
- Symptoms persist or recur
- Under 18 years old
- Over 60 years old
- Fever
- Discharge with unusual colour or odour
- Painful sexual in*******se
- Other urinary symptoms

Famciclovir is an antiviral medication used for treating cold sores. It is only effective if the patient is taking famciclovir within 24 hours of experiencing tingling/itching at the site of infection. Cold sores is normally self-limiting and resolve within 14 days without treatment. It can happen up to 3 times a year in certain patients.

Signs and symptoms:
-localised burning, itching, tingling or pain

Duration of cold sores: 7-10 days

Trigger Factors:
-exposure to UV light
-trauma to the area
-fatigue
-stress
-menstruation
-UTI (Upper respiratory tract infection)

Dosage:
Take 1500mg (3 tablets) as one-off dose during the tingling phase (within 24 hours of the outbreak of cold sores).

Who can take it?
Patient who is more than 18 years old who is not pregnant, not breastfeeding and not renal impairment/ immunosuppression. Pregnant or breastfeeding ladies are recommended to use aciclovir cream instead.

Adverse Effect:
Generally well tolerated but may cause nausea, diarrhoea, headache or fatigue.

Refer if:
- Immunosuppresipn
- Renal Impairment
- Older than 50 years old
- Younger than 18 years old
- Recurrent cold sores >3 times per year

The active ingredient of Chlorsig eye drops or eye ointment is chloramphenicol. It is an antibiotic to be used in bacterial conjunctivitis.

Bacterial conjunctivitis can spread from one eye to another and from person to person. In Australia, Chlorsig eye drops/ointment can only be supplied to a person who is older than 2 years old.

Symptoms of conjunctivitis:
-Sticky discharge (patient may find dried crust in the morning)
-Red or pink conjunctivitis
-Burning, gunky or gritty sensation in the eyes

Dose:
Eye drop: Administer 1 or 2 drops to the affected eye(s) every 2 hours for the first two days and then four times a day for the next 3 days.
Eye ointment: Administer 1.5cm into lower eyelid every 3 hours or at bedtime only if used in conjunction with the eye drops.

What to expect:
Symptoms usually improve within 48 hours of commencing the eye drops and it is advisable to continue using it for extra 2 days after symptoms resolved. Duration of treatment is 5 days.

Adverse effects:
-Stinging of the eyes
-Local allergic reactions such as swelling and redness of the eyes

Storage:
-Store at normal room temperature after opening and discharge it after 28 days of opening.

Lifestyle points:
-Do not share eye drops or ointment
-Do not share towels or face washers
-Do not rub your eyes
-Do not wear contact lenses
