After finishing medical school I was keen to do a masters in public health but fate lead me to be a paediatric anaesthesiologist.. a job I love! - - As an anaesthesiology resident I first thought that I’d have to give up my dream of ever doing anything related to public health because no one considers an anaesthetist physician when planning something related to public health! - - But I have come to realise that the resident me could not have been more wrong! We anaesthesiologists play a part in patient safety, quality control, policy and as a result global health. - -
Patient safety for me as an anaesthesiologist who was trained in a high resource exclusive paediatric hospital and now working primarily in low resource settings is a big concern. I cannot stress how important it is to follow guidelines, have checklists, have the right equipment (irrespective of how simple the case may seem) and the biggest of all trained staff! While I try to inculcate all of this and more in my practice and train the staff I have around me in patient safety some days are more challenging than others! - -
Let’s try to create awareness amongst patients and medical professionals about the importance of safety and quality in anaesthesia particularly in the developing and underdeveloped world! -

Another day Another paediatric cataract. This case was a preterm child for congenital cataract. When I tell people who are not part of the medical fraternity that I visit ophthalmic centres to anaesthetise children for eye surgery and we do a lot of paediatric cataracts, invariably the reaction I get is - “Cataract in a child? Really?” Most people seem to associate cataracts with older adults or a consequence of aging. But cataracts can occur in children as well. They maybe present at birth or develop after and could be present in one eye or both the eyes. - Some statistics I found on the American Academy of ophthalmology (AAO) website are: •Cataracts are estimated to be present in approximately 1 to 15/10,000 children worldwide, accounting for 5%–20% of childhood blindness (Foster et al, 1997).
•Globally, about 200,000 children are blind from bilateral cataracts worldwide (Foster et al, 1997; Medsinge et al, 2015).
•About 20,000 to 40,000 new cases of bilateral congenital cataracts are diagnosed each year (Apple et al, 2000). Although I am not an ophthalmologist myself I work closely with a lot of ophthalmologists and feel the need to create awareness about this treatable cause of blindness.

Happy New year 2020!! While the world celebrates and brings in the new year let’s thank the healthcare providers (doctors, nurses, anaesthesia & surgery technicians) and the hospital support staff, without whom the OR and emergency rooms cannot function, for leaving their friends & families and working through the night and into the new year for their patients!

HEALTH EQUALITY OR EQUITY? IS THERE A DIFFERENCE? WHICH DO WE WANT?
The above illustration best describes the two terms. It shows 3 people of different heights peering into a cricket field. Equality will be giving each of them a box of the same size to stand on so they can view the game.This wouldn’t help the shortest person who still won’t be able to watch the game and would not benefit the tallest person who would be able to view it anyway. Equity means we give each of them what they actually need to watch the game.
As per the World Health Organization , “Equity is the absence of avoidable or remediable differences among groups of people, whether those groups are defined socially, economically, demographically, or geographically. Health inequities therefore involve more than inequality with respect to health determinants, access to the resources needed to improve and maintain health or health outcomes. They also entail a failure to avoid or overcome inequalities that infringe on fairness and human rights norms.” Therefore, equity can be achieved by giving people what they need as per their circumstances. This helps us to direct our resources where they are needed.
