In the presence of deep decay, it is possible to resort to the direct pulp capping procedure, which allows to avoid root canal therapy. Thanks to the latest generation materials such as the biodentine putty NeoPutty, it is possible to apply the material in direct contact with the dental pulp and proceed with the direct reconstruction in a single session.

In presenza di una carie profonda si può ricorrere alla procedura di incappucciamento diretto della polpa dentale che permette di evitare la terapia canalare. Grazie a materiali di ultima generazione come il putty di biodentina NeoPutty è possibile applicare il materiale a diretto contatto con la polpa dentale e procedere alla ricostruzione diretta in un'unica seduta.

In some cases of root canal retreatment we can find fractured instruments such as lentulus: the removal of the latter is mandatory in order to proceed with the complete root canal shaping and must be performed with careful and delicate use of ultrasound to avoid fracturing the lentulus into multiple portions. Once the fractured instrument has been removed from the canal, retreatment can be completed.

In alcuni casi di ritrattamento canalare possiamo trovare degli strumenti fratturati come i lentulus: la rimozione di quest ultimo è obbligatoria per poter procedere alla completa sagomatura canalare e va eseguita con un attento e delicato uso degli ultrasuoni per evitare di fratturare il lentulo in più porzioni. Una volta rimosso dal canale lo strumento fratturato si può completare il ritrattamento.

The introduction of new heat-treated alloys has improved the effectiveness of the preparation of narrow and curved canals; the Slim Shaper PRO are "dual wire" combining two alloys with different heat treatment in the same instrument thus allowing a greater cutting capacity in these canals. The tip is more resistant to fracture and any stretching phenomenon is carried over to the part of the instrument with a greater martensitic share of the alloy.

L'introduzione delle nuove leghe trattate termicamente ha migliorato l'efficacia della preparazione dei canali stretti e curvi; gli SLim Shaper PRO sono "dual wire" combinando due leghe a diverso trattamento termico nello stesso strumento permettendo così una capacità di taglio maggiore in questi canali.
La punta risulta più resistente alla frattura e l'eventuale fenomeno dello stiramento si riporta sulla parte dello strumento con una maggiore quota martensitica della lega.

The literature describes the presence of the mesial median canal in a percentage equal to 5.7% of the lower first molars; failure to locate and treat this canal (which in most cases is confluent) can lead to failures of the endodontic treatment due to the lack of disinfection of the bacterial load present in it.

La letteratura descrive la presenza del canale mediano mesiale in percentuale pari all' 5.7% dei primi molari inferiori; il mancato reperimento e trattamento di questo canale (che risulta nella maggior parte dei casi confluente) può condurre a fallimenti del trattamento endodontico per la mancata disinfezione della carica batterica in esso presente.

The patient comes to our attention for pain in the first quadrant; the previous therapies performed on teeth 1.4. and 1.5. are not congruent at an endodontic and conservative level. In a single session, endodontic retreatments and the reconstruction phase are performed. After 12 months, comparing the cbct, a clear improvement is noted with an almost total restoration of the health of the bone tissue.

Il paziente giunge alla nostra osservazione per dolore nel primo quadrante; le pregresse terapie eseguite sui denti 1.4. e 1.5. risultano non congrue a livello endodontico e conservativo. In una singola seduta vengono eseguiti i ritrattamenti endodontici e la fase di ricostruzione. A distanza di 12 mesi confrontando le cbct si nota un netto miglioramento con un ripristino quasi totale della salute del tessuto osseo.

Deep Margin Elevation should be applied with caution respecting three criteria: capability of field isolation, the perfect seal of the cervical margin provided by the matrix, and no invasion of the connective compartment of biological width.
As the literature suggests, it is currently a valid technique that allows us to avoid surgical lengthening of the clinical crown in the presence of deep carious margins.

La rilocazione del margine profondo deve essere applicata con cautela rispettando tre criteri: capacità di isolamento del campo, perfetta tenuta del margine cervicale fornita dalla matrice e nessuna invasione del compartimento connettivo di ampiezza biologica.
Come la letteratura ci suggerisce è ad oggi una tecnica valida che permette di evitare l'allungamento chirurgico della corona clinica in presenza di margini cariosi profondi.

As reported in this beautiful article by Italian authors in the case of root canal filling with bioceramic sealer healing is rapid with repair/regeneration of lost tissues with cementum and new bone trabeculae; the histologic results illustrate the highly biocompatible and bioactive nature of the tricalcium silicate-based materials used in treating these cases.
The clinical observation that I have been able to perform over the years is a rapid healing without any cement extrusions being reabsorbed even after long follow-up periods.

Come riportato in questo splendido articolo di autori italiani nel caso di otturazione canalare con cemento bioceramico la guarigione è rapida con riparazione/rigenerazione dei tessuti persi con cemento e nuove trabecole ossee; i risultati istologici illustrano la natura altamente biocompatibile e bioattiva dei materiali a base di silicato tricalcico utilizzati nel trattamento di questi casi. L'osservazione clinica che sono stato in grado di eseguire nel corso degli anni è una rapida guarigione senza che l' eventuale estrusione di cemento venga riassorbita anche dopo lunghi periodi di follow-up.

Element 1.4. it has a correctly executed inlay but is necrotic and painful on percussion, the canals are highly atretic and with multiplanar curvatures.
The endodontic treatment is carried out through the inlay with a minimally invasive approach using the Slim Shaper Zarc; the follow-up CBCT one year later shows the case has recovered.

L'elemento 1.4. presenta un intarsio correttamente eseguito ma risulta necrotico e dolente alla percussione, i canali sono fortemente atresici e con curvature multiplanari.
Il trattamento endodontico viene compiuto tramite l'intarsio con approccio mini-invasivo utilizzando gli Slim Shaper Zarc; la cbct di controllo a distanza di un anno mostra la guarigione del caso.
