***** EYEBROW/EYELID RECONSTRUCTION AFTER SKIN CANCER SURGERY *****

This patient had a long-standing growth in his eyebrow hairs that he thought was a mole. The lesion was biopsied and found to be a basal cell carcinoma. After skin cancer removal, this patient required reconstruction of his eyebrow and upper eyelid.

Defects involving the and can be challenging for multiple reasons. In terms of function, this particular defect required careful reconstruction to ensure that the upper eyelid could still open and close normally. Cosmetically, it is critical to place incisions at aesthetic borders is important to create a less conspicuous scar.

In this patient, a local flap was developed to conceal the incisions as best as possible, and properly align the brow hairs

He is healing well and happy to be free of skin cancer.

This wonderful patient was bothered by the appearance of her lower eyelids. She noted under eye bags that were present no matter how much she slept the night before.

This patient presented with the complaint of vertical lip lines, often harshly named smokers lines. These lines form for a variety of reasons that vary from genetics, repetitive mouth movements such as drinking from a straw, soft tissue atrophy, dentition changes, and others. They are also sometimes referred to lipstick lines as the product can run into the crevices and smear.

She was NOT looking for a significant increase in volume and wanted it to look NATURAL.

Shown immediately after one syringe of Restylane Kysse she now has improvement in those vertical lip lines and more hydrated lips. She was very happy with the result 😊

If there are true under eye bags (prolapse of orbital fat), then under eye filler is almost NEVER the solution.

Prior filler can result in prolonged swelling and bump or cyst formation. Important to attempt to dissolve it before surgery.

This beautiful patient was bothered by puffy under eyes/ bags that created the appearance of dark circles even after a full night’s rest. She felt great and was still very active, but felt that this was a feature that didn’t match her energy level and was not able to cover it up with any kind of makeup. Attempts at under eye filler were unsuccessful and caused excessive swelling. The under eye filler was dissolved before surgery.

To improve her lower eyelid appearance without hollowing the lid cheek junction, lower blepharoplasty with fat repositioning was performed. This was done all from the inside of the lower eyelid. Light laser ⚡️ resurfacing was performed for improvement in lower eyelid skin texture.

Note the improved transition from her eyelid to cheek. The goal of this and any plastic surgery is “better” not “different”. She and I agree this was achieved 😊

Results are shown a few months later and will continue to improve!

She is happy with her results and I am very happy for her!

IMPROVING EYELID AND BROW SYMMETRY WITH EYELID SURGERY/BROW SURGERY👇👇👇

This beautiful patient was bothered by heavy upper eyelids and asymmetry. This made her look more tired than she felt and very difficult to wear mascara. Her previously radiant blue 👁️ 👁️ were disappearing.

Patient with paralytic ectropion repaired with eyelid/cheek lifting.

This wonderful patient was referred for eye pain, irritation, and tearing 😭 from a severe out turned lower eyelid . She developed this from a rare type of cancer that also caused double vision. She was unable to close her eye and this caused pain, eye watering, and disfigurement.

She underwent to repair her lower eyelid by supporting the cheek vertically and tightening the lid horizontally. She now notes that her irritation and tearing 😭 are resolved 😀!

She is very happy with her result!

The medial canthus is a complex area to reconstruct due to the meeting of several aesthetic subunits of the face (eyelid, nose, and cheek), the dynamic movement/contours that occur here, and the risk of functional deficits if not carefully performed (inability to close eye, pulled down lid, tearing, etc)

To repair this defect the patient had a combination of a facial flap and a full thickness skin graft. The flap was used to reconstruct the eyelid/cheek component of the defect and the graft was used for the nasal sidewall component of the defect.

At 3 months, she has a nice functional and cosmetic outcome. Most importantly the skin cancer is gone!!

BEFORE AND AFTER UNDER EYE “DOUBLE” BAG SURGERY 👇👇👇

This patient was bothered by under eye bags that created the appearance of dark circles even after a full night’s rest.

In this patient’s case, the puffiness was the result of “2 bags”. The first bag was from pr*****ed orbital fat. The second bag, below, is called a festoon or malar mound. This second bag, is not fat but more related to fluid retention and can fluctuate.

To improve her lower eyelid appearance from the bag caused by fat prolapse- lower blepharoplasty with fat repositioning was performed. This was done all from the inside of the lower eyelid.

To treat the second bag, or festoon, a custom laser resurfacing procedure was performed to tighten the skin with no cuts on the skin.

Results are shown at 2 months and will continue to improve!

She is happy with her results and I am very happy for her!

can present all over the body and the is no exception. This patient underwent reconstruction of a full thickness defect in her upper eyelid after Mohs surgery. She was missing skin as well as the structural component of the eyelid called tarsus (I often refer to this as the cartilage of the eyelid to patients). Prior skin removal from an upper blepharoplasty elsewhere made it a little trickier.

The full thickness component of the defect was repaired with a sliding tarsus flap and then a transposition flap was used to repair the skin component of the defect. (swipe to see surgery photos)

Upper eyelid reconstruction is less common and more difficult that a defect in the lower eyelid. This is due to typically less laxity and the zero margin for error for function as the upper eyelid is critical to protecting the eye.

She is shown only 2 months after surgery and will continue to improve. Form and function have been restored. I am so happy for her!

**** COMPLEX RECONSTRUCTIVE SURGERY OF THE NOSE/NOSTRIL/CHEEK****

This patient was referred for after for involving his right nasal ala, alar groove, medial cheek.

Defects involving the nasal ala and alar groove are complex. The ala lacks true cartilage so deeper defects require placement of a cartilage substitute, otherwise the external nasal valve (nostril) may collapse and make it difficult to breathe. The cartilage graft must then be covered by a vascularized flap (skin with blood supply).

You can see in the photo looking up into the nostril (“worm’s eye” view) that before repair the nostril is collapsed inward.

To rebuild the nostril support, I took a cartilage graft from his ear. The cartilage graft was then covered with an interpolated paramedian forehead flap. While this flap looks extreme and requires a second stage, it is necessary in cases like this to cover the cartilage graft with a vascularized flap and avoid distortion of the nasal alar groove. ➡️ to see the inset of the cartilage in the ala (nostril area) and coverage with forehead flap.

The forehead flap was separated 3 weeks later and the after photo is 6 weeks after flap division.

There is a small web at the lateral alar groove and this happens in the majority of cases where the defect extends across the alar groove and requires a third stage procedure to remedy. The patient was very happy already and elected to not have a 3rd stage contouring procedure.

He is healing very nicely and is very happy with the results! He can breath perfectly, has a well formed ala and alar groove (importantly, he is !

A transposition flap was used to repair the defect to keep incisions in the smile lines and avoid a skin graft.

Some patients have more fullness or more of a “bag” under their eyes. Some patients have more texture issues or wrinkles to the skin. Some patients have both.

To address both, the patient had #1 lower blepharoplasty with fat repositioning. This was done all from the inside of the lower eyelid with NO SKIN INCISION. She also had #2 laser ⚡️ resurfacing to the lower eyelid skin to help smooth and tighten the skin to address the texture component.

This wonderful patient was referred by her dermatologist after for reconstruction of her nasal inferior dorsum/supratip.

Caudal (inferior) nasal defects can pose challenges due to lack of local skin mobility and may require 2-stage surgeries (paramedian forehead flap) or skin grafts.
was performed in this patient in a single stage with a trilobe flap to spread the tension out over a greater surface area and recruit local skin to repair the defect. While the incision is longer in this type of repair, if carefully done it can lead to a nice result due to less tension on the leading edge of the incision.

She is shown 2 months after surgery and healing well.

SWIPE ➡️ to see defect photo and immediate reconstruction result. Reconstruction after Mohs surgery with free tarsal graft and local eyelid flap.

This wonderful patient was referred for reconstruction after removal of a basal cell carcinoma of her lower eyelid.

The skin cancer removal resulted in a full thickness defect of ~40% of the lower eyelid.

Several reconstructive options were discussed with the patient. She had minimal horizontal laxity which limited our options. One option would be a Hughes flap from the inside of her upper eyelid that is left connected and then separated 3-4 weeks later.

In this case, a free tarsal (“cartilage-like” part of eyelid) graft was taken from the inside of the upper eyelid and then a local skin flap was designed to cover the tarsal graft. These incisions were placed under the eyelashes to conceal the scar.

This allowed for a one-stage surgery that restored function to the eyelid and maintained cosmesis.

While this technique is not the best for every patient, in this case The patient was very happy with her result!

***** DEEP LASER ⚡️ RESURFACING FOR ORANGE 🍊 PEEL CHIN AND PERIORAL WRINKLES ***** This wonderful patient in her 60s was bothered by texture changes in her chin and wrinkles around her mouth and upper lip. #orangepeelchin #pebblechin #golfballchin develops from an overactive mentalis muscle. Early on, this can be modulated with #Botox or other neurotoxins. However, over time just like with repetitive movements in other parts of the face (in between the eyebrows/glabella with the 11 lines or the crows feet), these lines become etched in and are no longer responsive to Botox. In this case #scitonflorida deep laser resurfacing was performed to improve the texture that was present at rest (static wrinkle) without facial expression. Botox can be used in the future to decrease recurrence. #laserskinresurfacing has many different forms and shapes and can be confusing to interested patients. Ablative vs Non-ablative #laser , #fractional vs full field , differences in #downtime, #cost , #results . She underwent #deepresurfacing with #Sciton TRL Contour laser. We discussed multiple options and decided on this full field/fully ablative laser due to her goal in deep perioral wrinkle improvement.
