It is true that some off-label drug use is based on very unsettled science and has more risks. But medicine - and not just cancer care - involves lots of hard choices. And the more serious the disorder, often the more likely it is that for every right and wrong treatment choice there are many other practical decisions painted in shades of gray. I knew medicine only by its absence - specifically, the absence of a father growing up: one who went to work before dawn and returned in the dark to a plate of reheated dinner.

Taxes, well laid and well spent, insure domestic tranquility, provide for the common defense, and promote the general welfare. Taxes protect property and the environment; taxes make business possible. Taxes pay for roads and schools and bridges and police and teachers. Taxes pay for doctors and nursing homes and medicine. I had always wanted to become a neurologist, which is one of the most demanding vocations in medicine. Where do you stop, after all, with the brain? How does it function? What are its limits? The work seems unending.

Parents, of course, have concerns and 'say,' but they don't have the right to shield their children from knowledge. That is not a right, any more than they have the right to shield their children from healthcare or medicine. Republicans and others who are in anguish over the possibility of socialized medicine ought to have to explain their ideology to a mother with a sick newborn. They ought to have to explain how this nation can debate health care and not mention how abysmal ours is.

I will say that there is an inordinate amount of medicine in my novels, especially the first one. There are a lot of medical things that happen. A hip fracture, three different kinds of lung cancer, pneumonia, blood poisoning, and so on. From the beginning of time, we've told stories, Shamans and Medicine People, and not to be pompous about it, but I feel like that is the lineage I take down and where I come from. There is magic to storytelling.
