It gives me immense pleasure to share the news of launching of our NRI-App. It an excellent amalgamation of science n technology which would definitely prove to be of great help in newborn resuscitation. The app is on NNF algorithm for Neonatal resuscitation and was inaugurated at hands of Honorable Chief Minister of Odisha Shri Navin Patnaik at 35th annual convention of NNF 2015. This is the first such app in world and would be extremely helpful for those who attend deliveries.
This app is currently only on ANDROID. Type NRI and see for the logo.
Forthcoming version of this app will be with pictures, videos and voices.

Its a joint venture of Dr Deopujari Sir, me (Dr Chandrashekhar Fande) & Dr Vivek Shivhare.

Mystery solved! Culprit unmasked!!

28 days old neonate was referred to Healthcity Children's Hospital at 8pm with history of respiratory distress, feeding difficulty and failure to thrive. He had 2 admissions in 3 weeks span and was referred in v/o resistant pneumonia and had visited multiple practitioners but without relief.
Detailed history and thorough examination hinted at possibilities of either congenital heart disease or any lung malformation. Baby had significant failure to thrive , suck rest suck cycle, and feeding difficulty with exertional dyspnea. Baby was extremely irritable, febrile, toxic, with increased work of breathing,perfusion was good, maintaining saturation with minimal oxygen. Spo2 improved from 85 to 92% on oxygen.We did x Ray chest which showed normal parenchyma with cardiomegaly.
Suspecting a heart disease we did urgent 2 D echo of baby at within one hour of admission at 9 pm. On putting a probe on baby's chest the first and most remarkable finding was grossly decreased LV ejection fraction ( 5-10 %). ALCAPA ( anamolous origin of left coronary artery from pulmonary artery). No evidence of Coarctation of Aorta. This was the culprit for such a severe myocardial dysfunction.
ECG findings were suggestive of Abnormal q waves in left lateral leads suggestive of anterior MI.
At 10 pm, CVTC surgeon was called in to discuss possible treatment options with relatives.
What I want to emphasis through this case is,A baby is in agony since one month coming to you and you are giving him diagnosis in 1 hr with consultation of best persons in the field is really creditable job.

Operated gastroschisis ready to go home.

Last week I uploaded photos and summery of baby. We could successfully manage surgical and intensive care part but there was always concern of malabsorption. These babys almost have 60 to 80 incidence of malabsorption.

We started feeds and could reach full feeds within 5 days. Currently baby is on oral demand feeding and ready to go home.

Literature wise survival of gastroschisis with gangrenous bowel loops is very low but a skilled surgeon, hard working intensive care team and high level of nursing can change the outcome.

This is the best example.

Gastrochisis with Pregangrenous bowels : Managed successfully in Health City NICU.
A state of the art Neonatal surgery and Intensive care unit.
This newborn baby was referred to us from Madhya Pradesh on day 2 of life after taking so many consultations and very little hope. Bowel loops were already showing Pregangrenous changes. Our team has taken brave decision to go ahead with surgery after counselling relatives.
Generally what we think ,it is just a hernia,reduce it and close it! Simple!
But practically it is not that easy. Managing gastroschisis post operative is a tedious job. As intestines are outside the abdominal cavity, abdominal cavity is not well developed. If we try to push the contents inside and close it in one setting it can create two major issues,1. abdominal compartment syndrome,2.ventilation issue as bowel are going to push diaphragm up to hamper lung compliance.
So we decided to reduce the contents stepwise.
In fist step we partially reduced the contents and half kept out by covering it with polythene sheet ( as sialo bag was not available). Daily we were pushing the contents inside to some extent.
In second step ( after 5 days) bowel was completely pushed inside and abdominal wall closed.
Baby was on ventilator for total 10 days. Post operatively during final closure we have to struggle a lot for ventilation due to splintting of diaphragm. During this period we have to keep watch on hydration( insensible water loss was managed with almost 300ml/kg fluids) PICC line, asepsis and high level of nursing care. Baby was kept paralyzed for 10 days.
Baby was extubated to oxygen on day 10 . now baby is fine. Feeding started. RT aspirates decreased.
