h outcomes determined by social and environmental factors that occur outside the boundaries of a traditional health care setting, it’s a key component of OneCare’s strategy to invest in community-based population health efforts like its RiseVT program.

RiseVT Chittenden County is here to encourage healthy and resilient spaces where we live, work, play, and learn by celebrating and amplifying the great efforts in our communities. You can find us at some of you favorite local places and events across the region with engaging activities for all ages.

To start, RiseVT Chittenden County is focusing on our initial communities - Richmond, Huntington, and Bolton. We are inviting Vermonters from these towns to join RiseVT and make small changes that can make a big impact on their health. If you are in Chittenden and live outside of these communities, you can still join us for events! RiseVT is a community-driven movement to improve population health, working collaboratively with communities, organizations & initiatives to make The Healthy Choice, The Easy Choice. RiseVT is a statewide organization that is engaging Vermonters in every community. Launched in Franklin & Grand Isle Counties in 2015, RiseVT expanded to Chittenden County in 2018. RiseVT works across all sectors to help make the healthy choice the easy choice. This means we work with individuals, businesses, schools & classrooms, childcare providers, and municipalities
