We are currently looking for a Medical Assistant and Prior Auth specialist for our office.

MA: Willing to train the right person with any medical background (EMT, CNA, etc) or someone that is looking to go into the medical field (pre med, pre PA, pre RN etc). Must be available Tuesday-Friday, but FT is an option. Pay DOE.

Prio Auth Specialisr: must understand the insurance and prior authorization process. PT position. Pay DOE.

Myth - #2 continued (thyroid antibody levels are important in hypothyroid diagnosis and management)

See yesterday’s post on TPO ab.

Let’s discuss the other thyroid antibody level: the thyroglobulin antibody (Tg Ab).

Really NO need to check this lab for routine testing. (We as Endos check it in part of our monitoring of patients for thyroid cancer monitoring but otherwise we never need to check this).
- As an aside, definitely NO need to check the thyroglobulin level. We occasionally get referrals for a seemingly elevated thyroglobulin (Tg) level. Thyroglobulin is a marker of thyroid tissue. NO need to check this in a patient with a thyroid. The only time we monitor this is in a patient after a total thyroidectomy for monitoring for thyroid cancer recurrence.

In summary:
-TPO/Hashimoto’s/Thyroid peroxidase antibody level positivity is to be mostly expected in patients with hypothyroidism. (85-% have it)
-High TPO plus normal TSH = normal thyroid function/no treatment needed (except some exception in pregnant women or if tsh is borderline normal but some room to consider treatment).
-How high the TPO is doesn’t really matter.
-No need to check TPO more than one time (if at all).
-No clear diet/supplement to prevent progression to hypothyroidism (TPO levels may drop but this doesn’t translate to decrease in rate of progression to hypothyroidism).
-Overall, honest opinion of most Endos is that positive TPO antibody levels aren’t that big of a deal and we need to stop dramatizing this. Alternative practitioners/websites have created this ridiculous buzz around the Hashimoto’s term but it’s not worth the worry/stress. While all this may sound like
-we don’t care
-we are not testing enough things
-we are not thinking “deeper” about the thyroid, etc etc…
We are genuinely just looking for your best well being. We don’t want to waste your time worrying about something that frankly shouldn’t elicit this much concern.
-the “thyroid” industry has made a ton of money on books/supplements/testing/clinics preying on a fear of this antibody. And I’ll say it again… it’s actually not that big of a deal.
There, I said it … again and again and again.
