HIP MOBILITY💥😉

Mobilization of the hip joint capsule mobilization is paramount when helping patients with hip joint restrictions to improve their mobility. The more consistently the patient works on capsular mobilization between PT sessions, the sooner they can achieve better results.

This video demonstrates an example of assisted hip joint capsule mobilization🤘👇

Although banded hip joint capsule mobilization is effective, I prefer to do it after ultrasound guided dry needling, because I can target the small deep muscles like the gemeli or deep portion of the gluteus minimus. Those muscles attach to the hip joint capsule and are very stubborn during both manual release and banded mobilization, simply because they are very deep.

FOOT STABILITY🤩👇

If yoga toe and short foot exercises are not working to get your foot core to fire try paying attention to your first ray.

The first ray is the segment of your foot made up of your first metatarsal and the first cuneiform bones. It is located on the inner side of your foot behind the first metatarsal, which connects to your big toe and intersects the transverse and medial longitudinal arches.

I love the water bag for rehab exercises, but this post is not about the water bag. It is about your medial longitudinal arch and foot stability.

Eighty-five percent of foot stability depends on the stability of the first ray, and you don’t need a fancy water bag to check it.

👌Stand in a high lunge position and swing a kettlebell or a barbell plate diagonally across the front of your body. As you swing, push through with the first and fifth metatarsal heads of your rear foot, as though you are about to push off, but keep the metatarsals in contact with the floor. Try to maintain big toe support while rotating with the heel lifted, and try to align your knee slightly away from your big toe.

In this video, watch our therapist struggle to keep his big toe on the ground as he swings the water bag.

Big toe instability can be the cause of pain, not only in your foot, but anywhere up the kinetic chain to your knee, hip and low back. It can even spiral up towards the opposite shoulder.

If you can maintain your big toe on the ground and load it in that position, use that ability to make your foot even stronger.

What causes low back pain❓

💡The sacroiliac joint (SI) is the most common cause of low back pain, a fact that is commonly misunderstood in conventional medical pain management. The pain does not actually come from within the SI joint, but from the ligaments that surround it.

🤚If you have SI pain, a steroid injection will not help. The pain may be caused by laxity of the long dorsal ligament, the posterior sacroiliac ligament, or sacrotuberous ligaments. In addition, nerves may be blocked by adhesions or overly tight muscles. In some cases, one SI joint is lax and strained while the other is tight, or one may be lax while the other is hypermobile.

🏥At NYSNRehab, we use diagnostic ultrasonography to image the ligaments surrounding the SI joint and test them in motion.

In 1997, I spent an entire year working daily with the legendary Dr. Karel Lewit, and learned mobilization techniques from him. They are still valid today. When they don’t produce results, we use radial shockwave to relax the soft tissues surrounding the SI joint.

For hypermobile SI joints, we use a specialized approach developed by Diane Lee, a world-renowned expert in SI joint rehab. I spent a lot of time with her, learning about SI joint treatment. I highly recommend her book, The Pelvic Girdle.

If the Diane Lee approach doesn’t work we use prolotherapy, an extremely painful procedure that should not be substituted for physical therapy, but only complement it.

🏋️‍♂️BFR Training in the Early Stages Post ACL Reconstruction

After ACL reconstructive surgery, it is critical to fully rehabilitate the muscles and connective structures that surround and support the knee. Conventional therapy is a slow process, since the tissues need to fully heal before they can be sufficiently loaded to build strength. Traditional physical therapy protocols do not allow for heavy resistance training for several months.

Blood flow restriction therapy with low loads has been shown to improve quadriceps and hamstring strength and increase the rate of hypertrophy in adults. BFRT lets us get a jump on the strengthening process by allowing for loads at approximately 20% of 1RM, to promote tissue neogenesis and increase muscle strength in the early stages of recovery.

With BFRT, we are able to accelerate the recovery process, to return the athlete to the playing field as quickly as possible, with reduced risk of re-injury.

Your muscles are made up of two types of muscle fiber — Type I slow twitch and Type II fast twitch.

Low back pain is a common complaint of middle aged adults, and a herniated disc is frequently the cause. One of the more frustrating and challenging after-effects of childbirth is diastasis re**us abdominis (DRA), the separation of the left and right halves of the long re**us abdominis (RA) muscle that forms the six-pack.

While separation of the RA during pregnancy is normal and indeed necessary to accommodate the growing fetus, in some cases the two halves do not come back together after childbirth. This happens when the linea alba, the strip of fascial tissue that dissects the right and left RA, is overstretched and thinned. In some cases, the linea alba actually splits.

Measuring Blood Flow and Muscle Stiffness for Performance and Injury Prevention

For decades, the benefits of stretching, when, how often and how much, have been grist for the debate mill. Some exercise scientists think stretching is a total waste of time, some think it is beneficial when done in a specific context, and some feel it is critical to optimal performance and recovery, and should be an integral part of any training, warmup and recovery routine.

Regular static stretching gives your joints a broader range of motion and added mobility, but to be useful for athletic performance, mobility needs to be balanced with stability. Static stretching performed prior to a competition or intense exercise can reduce muscle stiffness and increase blood flow to the muscles and tendons, to potentially enhance performance and reduce the risk of injury.
