This patient came to us with a partially edentulous case and a history of recurrent tooth decay. Despite minimal vertical bone loss, their smile and function had gradually diminished, affecting not just esthetics but daily quality of life.

After a comprehensive evaluation and discussing all treatment options, including those to preserve existing dentition, the patient ultimately chose a full-arch implant-supported zirconia prosthesis. Their decision was influenced by considerations of long-term durability, reduced treatment time, fewer visits, and cost-effectiveness.

This patient lost a front tooth due to childhood trauma—an injury that led to decades of dental treatments, including multiple fillings, root canal therapy, and a crown that eventually failed.

This smile had been through a lot—chipped bonding, discoloration, and decay that made it hard to smile freely.

We crafted a solution that blended veneers and crowns, designed not just to restore teeth, but to elevate the entire smile. Every step was intentional—preserving natural tooth structure, dialing in the bite, and broadening the arch for better lip support and a more youthful frame.

This custom smile design was tailored specifically to the patient’s needs—balancing function, health, and aesthetic goals seamlessly.

And we’re not done yet—future phases will expand into the posterior teeth to refine the buccal corridors and complete the transformation.

Capturing accuracy in full-arch implant restorations—trust but always verify.

Shown here is the IO Connect gauge by TruAbutment, just one of the many techniques used to record implant positions. In a world of evolving workflows—traditional impressions, scan bodies, scan guages, and photogrammetry—the question remains: How are we verifying accuracy?

In this case, I used my implant provisionals to create an implant verification jig, which also allowed me to capture the ovate pontic sites that were carefully sculpted during healing.

We often chase the picture-perfect prosthetic outcome—beautiful esthetics, pristine occlusion, ideal implant positions. But sometimes it isn’t until a complication arises that we’re reminded of the importance of foundational prosthodontic principles.

📍 The foundational principles.

This full-arch zirconia prosthesis fractured. Naturally, we revisited the usual suspects:
🔹 Prosthetic design?
🔹 MUA interface?
🔹 Parafunctional habits?
🔹 Restorative space?
🔹 Occlusion?
🔹 FMA angle?

But what about:
➡️ Mandibular Flexure — did we allow for the mandible’s natural bending?
➡️ Denture Tooth Positioning — did we honor the principle of keeping teeth within the confines of the residual ridge?

In traditional complete dentures, we’re taught to avoid cantilevering teeth off the ridge — to stay in the neutral zone, to respect function. Yet in fixed implant cases, especially when chasing esthetics, we sometimes forget that physics still apply.

🦷✨ Anterior Implant Esthetics – It’s More Than Just the Crown

When restoring a smile in the esthetic zone, every detail matters — and it starts well before the crown is chosen.

👁️ The key to a natural-looking implant lies in understanding the patient’s gingival biotype, smile line, and the relationship between bone and soft tissue. A thick biotype may tolerate metal components, but a thin biotype? That demands precision and translucency — or you’ll risk grey show-through and recession.

🔩 Abutment choice is critical:
• Zirconia abutments offer high esthetics for visible areas
• Titanium works well in thick tissue or deeper placement
• Hybrid abutments (zirconia on titanium base) give you both beauty & strength
• Casted custom abutments allow for tailored emergence when anatomy is unique
• And for off-angle implants, Angled Screw Channel (ASC) options help keep the screw access in a hidden spot without sacrificing retrievability

👑 When it comes to crowns:
• Monolithic zirconia = strength (think bruxers)
• Layered porcelain fused zirconia = beauty (but handle with care)
• Lithium disilicate (e.max) = unmatched esthetics in low-load zones
• PFM still have their place in complex cases

Every choice we make — from material to margin, from screw vs. cement retention — influences how the restoration looks today and years from now.

When comparing the upper teeth to the lower teeth, you can see the difference in generic zirconia prosthesis — and you’ll instantly see the difference in aesthetics, translucency, and overall natural appearance. Motivated grandma seeking to match her beautiful soul with her smile! Initially wanted to only address the upper but after careful evaluation both arches needed to be addressed while restoring her lost vertical height. Single surgical visit she was able to walk away with teeth after implants were placed. Final teeth composed of monolithic zirconia with customized staining to match age appropriate grandma!

Born with missing lateral adult tooth that never came in and always feeling insecure about it hence the mustache. However finally found the courage to address the missing tooth with orthodontic treatment and implant placement. Customized abutment Porcelain fused to Zirconia Implant crown with custom staining to match the adjacent teeth. Soft tissue conditioning was performed with a highly polished provisional crown and transferred for the ceramist to replicate emergence profile.
