That's why our Choose Tomorrow program proactively reaches out to members to connect them to support before crisis. Through data insights, we identify members at-risk of su***de and connect them with care managers who reach out weekly to ask, "How are you?" and listen without judgment. This approach supports steady participation in life-saving care through ongoing personal connection while linking to resources, helping transform feelings of isolation into hope.
Early support can be crucial in su***de prevention, but many people experiencing suicidal thoughts lack access to the care and connection they need. Our Choose Tomorrow program uses data-driven insights to identify and connect with members at-risk of su***de to ensure they have access to vital care....

Healthcare goes beyond medicine. When Sinai faced leukemia, diabetes, and multiple surgeries, Coordinated Care connected her with senior care manager Dawn to help her navigate the complexities of her health challenges. Together, they addressed both her medical needs as well as education, housing, and getting around. Today, Sinai manages her own care while living independently.

More than 85,000 people in Los Angeles will have better access to care through two new initiatives: the L.A. County Field Medicine Program and the Skid Row Care Collaborative.

Physical activity is an important part of improving overall health, but one barrier many people face is a lack of access to fitness equipment. That’s why, in Douglas, AZ, Centene company Arizona Complete Health partnered with the National Fitness Campaign to provide a new outdoor fitness court. This gives residents the resources they need to get and stay healthy. Through partnerships with local organizations, they created a space where not only residents can exercise, but the whole community can stay active together.

Stephanie's 5-year-old son Noah has level three autism and needs constant supervision. Sleep is a struggle. Daily routines are challenging. Working with a care coordinator at Meridian of Illinois, a Centene company, Stephanie accessed resources like an adaptive bed, specialized equipment, and therapy services.
Now Noah sleeps through the night and is becoming more independent. For Stephanie, this progress has been life changing.
