"Can I still get pregnant with DOR?"

A diagnosis of Diminished Ovarian Reserve (DOR) can be emotionally challenging, but it does not mean the door to pregnancy is closed. It simply means that your ovaries are producing fewer eggs than average, which may affect the number of eggs retrieved during a fertility cycle but doesn’t necessarily impact your ability to conceive.

DOR refers to egg quantity, not egg quality... and pregnancy is still possible with fewer eggs, especially when guided by an individualized treatment plan.

While those with DOR may require different stimulation protocols or more cycles to achieve the desired outcome, many go on to have successful pregnancies. Understanding the condition, staying proactive, and working with a fertility team that tailors care to your specific reproductive profile can make all the difference in your journey.

Ways to Make IVF Affordable with PCOS

One key strategy for making IVF more affordable and more effective involves customizing the medication protocol to reduce unnecessary drug use without compromising outcomes. Certain PCOS patients benefit from a lower-dose stimulation protocol that incorporates Letrozole, an oral medication typically used to induce ovulation. Unlike protocols that rely solely on injectable FSH (follicle-stimulating hormone), adding Letrozole can help stimulate the ovaries more gently, reducing the risk of overstimulation.

This approach isn’t just biologically sound, it’s also cost-effective. By reducing the total amount of injectable medication needed, patients often see significant savings on drug costs. And beyond saving money, this modified protocol can sometimes lead to better egg quality, which is especially important for improving IVF success rates and reducing the risk of complications like ovarian hyperstimulation syndrome (OHSS).

For patients with PCOS, a personalized treatment plan that accounts for their hormonal profile, ovarian reserve, and financial considerations can make all the difference in their fertility journey.

41, Single & Unsure?
Here’s What Dr. Luk Recommends

For women in their early 40s who are single and haven’t yet frozen their eggs, fertility decisions can feel overwhelming. The biological clock becomes more pressing, and uncertainty about future relationships often adds an emotional layer of complexity. Still, options do exist, but timing is critical.

Dr. Luk encouraged viewers to reflect seriously on whether having a child is something they truly want. If the answer is yes, she said, it’s worth considering becoming a “single mom by choice”: a path many of her patients have taken and found deeply fulfilling. “They’re very happy,” she noted. “Obviously they would love a companion, but sometimes when you don’t have a partner and it’s something you want, you never want to lose that chance.” She emphasized the importance of a strong support system from family and friends to make solo parenting feasible.

For anyone in this situation, it’s important to consult with a fertility specialist right away. Fertility declines sharply after age 40, and delaying decisions can limit your options. Egg freezing, embryo creation, or even moving forward with donor s***m are all possibilities, but they require timely action.

When is it infertility?

Understanding when to seek fertility evaluation is essential for women who are trying to conceive. The timing of this assessment depends largely on age, and knowing when to act can significantly impact outcomes. While many patients start with hormone testing and ultrasounds to check their ovaries, Dr. Luk encourages a full evaluation when these time thresholds are met.

Early evaluation not only clarifies what’s happening inside your body but also opens up a wider range of treatment options, especially when time is a factor. And for women over 35, acting after just six months of trying is not rushing: it’s being proactive.

Egg Quality: Two Key Numbers Women Need to Know

Understanding egg quality is one of the most empowering steps a woman can take when considering her fertility health. While the topic can feel complex or even overwhelming, Dr. Janelle Luk simplifies it to two essential indicators: your age and how long you've been trying to conceive.

What’s more, these tests [such as AMH (Anti-Müllerian Hormone), FSH (Follicle Stimulating Hormone), and ultrasound evaluations of ovarian reserve] are often covered by insurance. Dr. Luk recommends consulting with a fertility specialist or OB-GYN to interpret the results and discuss appropriate next steps. These conversations can help women make informed decisions, whether they’re trying to conceive now or planning for the future.

Proactively checking your fertility health doesn't mean rushing into treatment. Instead, it provides clarity, and potentially more time, to make the decisions that align with your personal and professional life.
