What is the diagnosis?
Avascular necrosis of the femoral head.

📌Early X-rays may be normal or show subtle sclerosis; MRI is key for early diagnosis.

📌Look for the classic crescent sign on radiographs in intermediate stages, and eventual collapse with joint space narrowing in late stages.

📌Ficat and ARCO classifications guide staging and management.

🔨 If AVN is detected early (pre-collapse), imaging guides conservative management (e.g., protected weight-bearing, medication) or hip-preserving surgeries like core decompression. Once femoral head collapse occurs, imaging dictates the need for reconstructive procedures or to plan joint replacement surgeries.

The claw sign on diffusion weighted imaging refers to a pattern of hyperintensity seen in degenerative changes in the spine (Modic type 1): well-defined, paired band-like regions of restricted diffusion affecting two contiguous vertebral bodies resembling a claw surrounding the disc space on sagittal images.

💎 The presence of the claw sign is highly predictive of degeneration and has a high negative predictive value against infection.

💎 In contrast, diffuse or amorphous DWI hyperintensity, absent a claw morphology, predicts infectious spondylodiskitis.

Rheumatoid arthritis can quietly creep beyond the peripheral joints and into the spine—and when it does, the consequences can be serious. This case shows atlantoaxial instability with cord compression at the foramen magnum.

Chronic inflammation leads to ligamentous laxity and erosion of the odontoid and transverse ligament, allowing the atlas to slip forward over the axis. The result? Stenosis at the cervicomedullary junction, and potential for neurological symptoms or even sudden death in severe cases.

On MRI, look for pannus around the dens, anterior subluxation, and cord indentation or signal change. Always check the craniocervical junction in known RA patients with neuro symptoms.

There isn’t a single study in decades of CT use that directly links scans to cancer. As radiologists, it’s our responsibility to help patients and clinicians better understand the actual risks and benefits of imaging that uses ionising radiation.

☢️ Radiophobia is also not without risks!

That said, every CT scan must still be carefully justified—absence of conclusive evidence is not a license for unnecessary imaging.

For context, here’s an excerpt from ACR’s statement on this JAMA paper.

“The theoretical radiation risk proposed by Smith-Bindman, et al, is consistent with prior statistical modeling studies that are not based on actual patient outcomes. 

There are no published studies directly linking CT scans (even multiple CT scans) to cancer. Medical imaging exams (including CT) are a primary factor in declining cancer death rates, and are directly linked to decreased hospital mortality ratesand greater life expectancy.”
