「肺癌OligometastasesおよびOligo-recurrenceに対する外科治療の現状」東京医科大学・呼吸器外科 嶋田 善久先生の肺癌誌（日本肺癌学会公式雑誌）に掲載された和文総説です。
肺癌術後の再発時などにSBRT（体幹部定位放射線治療）も役立ちますが、根治性を高めるには手術の方がよいことも多いです。とくに昨今は、縮小手術で侵襲性も小さい手術が可能になっています。以下、お読みいただけますと勉強になるかと思います。

「先生のこの英文総説論文。日本以上に海外に影響を一番に与えたね。Oligometastasesがここまで世界で有名になったのは、この論文のおかげでもあるね」。とある著名な研究者の方から言われた言葉です。
たいへんありがたく思っております。
この英文総説論文は、臨床放射線に2012年に和文総説として公表した（再利用許可を得ています）日本語訳に相当する論文の元になったものです。本日現在、他の論文にて255回の引用を受けています。さらに別の統計方法（google等では、300回以上にもなっています）。多くの世界中の研究者の先生方はじめ一般の方々にも読んでもらえてたいへん嬉しく思っております。
Figureがあって分かりやすかった。というご意見をたくさんいただきました。Oligo-recurrenceというのは、再発時StageIVです。提案した当初は、初発時StageIV（sync-oligometastases（2012年に Niibe Y, Chang JYで提唱）やHellman先生、Weichselbaum先生らが提唱当初のOligometastasesの概念に近い状態）の方が予後が良いのでは ?、ということも言われましたが、その後の研究成果で、Oligo-recurrenceの方が多くの場合、Sync-oligometastasesより予後が良いことが証明されてきました。
全身療法と局所療法とOligometastasesの関連図もこの時点での予想としてはとても秀逸、とご評価いただいたりもしました。
この英文総説は、私自身も魂を込めて執筆しました。さらに幸運なことに、この英文論文を "Accept" と判定していただいた先生がJpn J Clin Oncol誌（IF: 3.019) に当時でもいらっしゃったことです！
「よい研究でも公表するには、理解者が必要」、と若い頃言われましたが、本当にそうだと痛感しております。
2010年1月公表（実質執筆は、2009年末）の英文総説ですが、歴史的価値もあり、今でも生きている内容のものと思いますので、是非、再度ご一読いただけますと幸甚に存じます。
* Sync-oligometastasesやOligometastasesの個数は 1-5個という定義は、2012年の NIibe Y, Chang JY. Pulm Med, 2012; 2012: 261096. で行っています。

第35回 高精度外部照射部会学術大会が2022年3月5日（土）～ Web開催されます。
シンポジウム 1 「オリゴメタスタシスへのSBRTは標準治療となるか」のセッションは、きちんとした Oligometastasesの日本での歴史を反映したセッションになるでしょうか？ （公社）日本放射線腫瘍学会研究課題で行った「肺OligometastasesへのSBRTの全国調査研究」で新部が研究代表者で、研究事務局は東北大学、その他主要な班員のすべてが座長、演者から外れています（論文の共著者になっていただきました先生方も1名も入っておりません）。
また、座長の鹿間直人先生は、2006年に厚生労働省小口班にて、
当時、世界初の "Oligo-recurrenceへのMDT（Metastases-Directed Therapy）の第II相試験" を新部がドラフトを提案させていただいた際、最前列で、「こんなのやって、仮にうまくいったら俺たち（鹿間先生等々）が平伏すとでも思っているのか！」 と厳しく、激しく、大声で 100名くらいいる参加者の前で罵倒していました（結果、世界初の第II相試験は却下となりました。班長の小口先生（現：がん研究会有明病院 副院長）も 却下となり、とても残念がっておりましたことも鮮明に覚えております）。私としては、研究は、もちろん「勝ち負けでなく、全ては患者さんのため（All for Patients）」、と思っていましたし、今も思っております。この発想自体にびっくりしました！！
さらに、日本放射線腫瘍学研究機構（JROSG）にて、新部がOligometastasesの放射線の多施設共同研究を実施したい旨、以前申しましたら、鹿間先生から「JROSGでは、一切、今後もOligometastasesの研究をしません」と同機構理事の立場でおっしゃっていました。
鹿間先生がどのような座長をされるのか（過去のいきさつへの懺悔があるのか。私は鹿間先生の性格からは、「ない」、と感じますが）視聴させていただきます。
演者の先生方にも、きちんとした日本が世界の先頭を走ってきたOligometastases、Oligo-recurrenceの歴史をふまえ、ご発表いただきたく存じます。
* 鹿間先生は 2006年当時は、研究班等でも、かなりハイテンションな先生でしたが、懺悔の弁はないにしても、今は落ち着いた座長をしていただけるかと予想していますし、願っております（冷静で公平な座長をお願い致します）。

東北大学 山本先生が筆頭著者で、新部がSecond author、東北大学 神宮教授がLast author のこちらの論文は、掲載誌も評価はされている雑誌ですが ( BMC Cancer: IF 4.430)、一般的な雑誌の評価以上に重要な世界初の知見を示しています。
「肺Oligometastasesに体幹部定位放射線治療 (SBRT) を実施すると、生命予後が延長するのには、SBRTにより、肺転移巣が制御されることが寄与している」という事実をランドマーク解析をして証明しました。これは、転移巣の制御が生存率向上に寄与するという世界初の証明です！
少々専門的で分かりにくいかもしれませんが、今までのがん治療の常識を覆す画期的な原著論文です。
