I had an absolute blast connecting with so many passionate and purpose-driven OT practitioners this weekend! 💛💡

This year, I was honored to speak on pelvic health again....busting myths, sparking conversations, and reminding everyone that OTs belong in this space. 🧠💪🏽🩺

One of the biggest highlights? Getting to learn from and hang out with the incredible Dr. Arameh Anvarizadeh! 🔥 Her keynote was nothing short of inspiring.... powerful, empowering, and deeply rooted in equity and leadership. 💬👑

Feeling so grateful for this community, for the energy in every session, and for the opportunity to keep sharing what I love with people who care just as much. 💥

OTs: this is our lane, too. Few things are more essential to daily life than peeing, pooping, and intimacy.
You don’t need permission to belong here. You already do.
Your unique lens is needed. Your clients are waiting.

This is occupational therapy at its deepest level.

I’m so excited for the upcoming Level 1 Functional Pelvic Health Practitioner™ Labs!

Very soon, 90 incredible OTs will step into this next transformative phase applying everything they’ve learned from the most comprehensive, OT-rooted pelvic health program out there.

This is the only 5-month program that gives OTs a truly guided, step-by-step path from newbie to confident pelvic health practitioner combining over 20 hours of online content, mentorship, and now, this robust hands-on lab experience.

Led by the amazing Lisa Loveless and supported by our incredible team of 2 in-person TAs and 6 online TAs, these students are about to gain the confidence and skills to assess, treat, and empower their pelvic health clients like never before.

What excites me most is watching everything come together — seeing knowledge transform into skill, and skill transform into confidence. After the lab, students will complete their skills-based assessment to officially earn their Level 1 Functional Pelvic Health Practitioner™ designation.... a testament to their growth and dedication.

I think it’s time to rethink how we talk about the internal exam in pelvic health.

The truth is, healing starts with creating a space where our clients feel safe, empowered, and fully in control of their own bodies. If the nervous system doesn’t feel safe, no deeper work can begin.

In the latest update of OT Pioneers, we’ve redefined how we approach internal exams and pelvic health, empowering therapists to offer a more client-centered, consent-driven experience that prioritizes safety and choice every step of the way.

Many still refer to the internal exam as the “gold standard,” but there are actually FOUR equally valid ways to work with pelvic health.

And here’s the key: each approach enables the client to choose what feels right for them. Whether they remain fully clothed or opt for an internal exam, each path is respected and the client is always the one in control.

When presenting the internal exam as an option, it’s critical that we make all choices clear and explicit. Clients must know they have the power to give—and revoke—consent at any time, and that all options are valid.

The true “gold standard” is the one that helps the client feel safe and regulated in their nervous system.

No longer should an internal exam be reduced to a checkbox or a quick nod of agreement. Instead, we need to provide clear details: where we might touch, for how long, and how clients can stop at any point, whether verbally or with a simple gesture.

Ask: Does any part of this feel uncomfortable for you? And...
Are you confident in your ability to communicate that?
You might also offer, “If using your voice feels hard, another option is to simply raise your hand.”

Why does this matter? Because if the nervous system isn’t regulated, even the most finely-tuned exercise program or manual therapy will fall short.
This is the care every person deserves—full agency and empowerment over their pelvic health journey.

Every day, OTPs work with clients whose pelvic health impacts their participation in meaningful activities.

Think about your current caseload. How many clients mention bathroom struggles, sitting discomfort, or intimacy concerns? How often do we hear 'I just hold it all day at work' or 'I can't sit through a movie anymore'? These aren't just medical issues - they're occupational barriers. It's time we expand our lens and start asking the questions that help our clients participate fully in life.

This doesn't mean every OTP needs to do internal exams or become a pelvic specialist. It means understanding when pelvic health is impacting function, knowing basic anatomy and mechanics, asking appropriate screening questions, and making informed referrals. It means recognizing that toileting, sexual health, and core stability aren't separate from occupation - they ARE occupation.
