We recently had the privilege of being part of the multidisciplinary team during an awake craniotomy performed by Dr Wiese, Neurosurgeon at Kloof Mediclinic. 🧠
An awake craniotomy allows surgeons to monitor and protect essential brain functions during surgery by actively engaging the patient.
As Occupational Therapists, our role involved completing a thorough pre-operative assessment of the patient’s motor skills, sensory function, and basic cognition to establish a clear baseline.
During the procedure, we re-tested these functions in real time, providing immediate feedback to assist the surgeon in identifying and preserving critical areas of the brain.
This collaborative approach helps optimise surgical outcomes while prioritising patient safety and functional independence.

As this year comes to an end, we pause—grateful, reflective, and deeply moved 🤍
This year held both light and heavy moments. There were days of joy and pride, and days of tears, loss, and quiet strength. Through it all, we grew, we learned, and we were reminded of the privilege it is to do this work!
Being named Best Occupational Therapy Practice in Pretoria, was a truly unforgettable milestone for us and an honour beyond words 🏆. Beyond the recognition, it has been a profound privilege to help elevate Occupational Therapy and to advocate for our profession, using this platform to highlight the impact, value, and heart of the work we do every day. Yet, the true reward has always been the lives we’ve been allowed to touch. From working in schools and growing alongside the children who inspire us daily, to supporting clients young and old through challenges, milestones, and breakthroughs.
We’ve cried with families in moments of loss, celebrated victories big and small, and stood beside one another, alongside amazing specialists—learning, supporting, and growing together.
Thank you to every child, every family, every school, and every professional who trusted us with their story.
As we look ahead, our hearts are full of hope—for healing, connection, growth, and reaching even higher heights with a more meaningful 2026 ✨

🌟 Shoulder AROM After Flap Surgery 🌟

Recovering shoulder movement after a flap surgery takes time, consistency, and the right kind of graded activities.

Here is 15 seconds of inspiration for active range of motion (AROM) tasks that build mobility, endurance and functional strength — without overloading healing tissue. 💪

Here’s what we used:
👕 Large washing pegs on a curtain – encouraging shoulder flexion & reach in a functional way.
🪟 Towel wiping on a window – great for controlled circular AROM and endurance.
🧠 Cognitive game with shoulder movements – challenging both the body and the brain to improve dual-tasking.
🍼 Bottle slides on a table – a gentle way to strengthen using graded resistance and to improve shoulder glide patterns.

These activities may look simple, but each one targets:
✔️ Controlled shoulder movement
✔️ Functional reach
✔️ Endurance building
✔️ Safe progression after flap surgery
✔️ Confidence in using the arm again

UTILISING THE SCHOOL PLAYGROUND 🛝
How can we use the school playground to work on gross motor skills?
Let's show you!
1. Balance with heel-toe walking on a line
2. Muscle tone, strengthening the body and praxis (motor planning) by climbing / sliding on the jungle gyms.
3. Vestibular stimulation, proprioceptive input and tactile integration, by sliding, dping monkeybars and crawling through the sandpit.
4. Play 'the floor is lava' where they plan a route from point A - B without touching the floor, to work on planning
Additional benefits:
- nature based sensory integration
- cardiovascular endurance
- stimulating the lymphatic system
- boosting seratonin production (improved mood 😜)
