๐ Antibiotic Stewardship: Essential Counselling Points for Optimal Patient Outcomes๐
Antibiotic counselling is a critical intervention for ensuring therapeutic efficacy, maximizing adherence, and minimizing adverse effects and resistance. As healthcare professionals, our concise and accurate advice directly impacts patient safety.
Here are key evidence-based counselling points for common antimicrobial agents:

1. Administration: With Food vs. Empty Stomach ๐ด
Take WITH Food: To mitigate GI upset and enhance absorption.
Metronidazole, Nitrofurantoin, Cefuroxime.
Take On An EMPTY Stomach: For optimal absorption (1 hour before or 2 hours after a meal).
Azithromycin (Zithromax) capsules, Flucloxacillin, Ampicillin , Fosfomycin.

2. Absolute Alcohol Avoidance ๐ซ๐ท
Metronidazole, Tinidazole, Co-trimoxazole: Counsel patients to avoid alcohol during and for at least 72 hours after the last dose to prevent a disulfiram-like reaction (nausea, vomiting, flushing, tachycardia).

3. Drug-Food Interactions: Dairy & Antacids ๐ฅ
Fluoroquinolones (Ciprofloxacin, Levofloxacin) & Tetracyclines (Doxycycline): Cations (Caยฒโบ, Mgยฒโบ, Alยณโบ, Feยฒโบ) in dairy, antacids, and supplements can chelate these drugs, severely reducing absorption. Advise taking these antibiotics 2 hours before or 4 hours after such products.

4. Benign Urine Discolouration ๐ฝ
Inform patients that harmless urine discolouration (often dark or reddish-brown) is a common side effect of:
Nitrofurantoin, Rifampicin, Metronidazole, Phenazopyridine.
This prevents unnecessary anxiety and alarm.

5. Contraindications & Use in Special Populations ๐คฐ
Pregnancy Category D/X: Emphasize strict contraindication.
Tetracyclines (Doxycycline): Risk of tooth discoloration and inhibition of bone growth in the fetus.
Aminoglycosides (Gentamicin): Risk of ototoxicity (8th cranial nerve damage).
Fluoroquinolones (Ciprofloxacin): Associated with arthropathy in animal studies.

6. Essential Hydration & Sun Protection ๐งโ๏ธ
Hydration: Crucial with Doxycycline to prevent pill-induced esophagitis and with Clindamycin to counter dehydration from potential GI effects.
Photosensitivity: A serious adverse effect. Advise strict sun avoidance, protective clothing, and high-SPF sunscreen with:
Tetracyclines (Doxycycline), Fluoroquinolones (Cipro/Levofloxacin), Sulfonamides (Bactrim).
