​Panic attacks, those sudden, intense surges of fear often accompanied by distressing physical symptoms, can be a terrifying experience. For many sufferers, the immediate instinct is to seek medication for relief. It is understandable at tge first time it happens. One needs to rule out any serious medical conditions. But when it recurs and persits, doctors themselves advice a psychological solution as the right choice.

In course of my three decades of clinical practice in Dubai, I have been helping scores of patients suffering from panic attack and helping them manage it without medication. ​The CBT- based Brief Therapy model which I put together is rooted in the following understanding: While panic attacks feel overwhelmingly physical, their origins lie in the learned responses and cognitive distortions. Mis-interpretations of one's bodily sensations is a significant factor. In my view, panic, in essence, is the body's natural "fight or flight" mechanism gone awry, suddenly triggered at odd times. Instead of long-term interventions, I have put together a set of evidence-based, targeted strategies. This method equips patients with the tools to de-escalate panic and prevent future episodes. This brief therapy model typically involves several key components. First, I help patients understand the physiological mechanisms behind a panic attack, demystifying the frightening symptoms. I help them get convinced that the symptoms are not life-threatening. This psychoeducation alone can significantly reduce anxiety. ​Next, I introduce practical, evidence-based techniques. Progressive relaxation using deep diaphragmatic breathing, is a cornerstone, which teaches patients to regulate their breath and activate the parasympathetic nervous system, which calms the body. I employ cognitive re-structuring to guide patients in to identifying and challenging the catastrophic thoughts that often accompany panic, replacing them with more realistic and reassuring interpretations.

​Exposure techniques, conducted gradually and safely, are another vital element. By gently exposing clients to the sensations or situations they fear, I help them habituate themselves to these triggers and realize that their fears are unfounded. This process re-teaches the brain that these sensations are not dangerous.

​The beauty of this brief therapy is the empowerment it creates within the patient. Patients quickly learn that they possess the internal resources to manage and overcome panic by applying the tools and techniques learned during the sessions. They gain a sense of control over their bodies and minds, often achieving significant relief and a renewed sense of freedom. All this is achieved without the need for pharmacological intervention.
Consistently postive outcomes is what makes this treatment model reliable. It is highly rewarding for me to be instrumental in transforming the lives of those grappling with panic attacks in the Gulf region.

Pre-existing personality disorders is one of the major factors in couple conflicts. Paranoid Personality, Borderline Personality and Narcissistic Personality are the most disturbing in couple relationships. Often the underlying mental health conditions remain undiagnosed, or even noticed as the key factor. A seasoned professional can identify such disorders through clinical interviews or psychometric tests.

In treating couples with a Narcissistic partner, it is impotant for the therapist to ​distinguish between
'standard' high-conflict marriage issues and Narcissistic Personality Disorder (NPD)?" We need to realize that a standard compromise-based therapy often fails or even backfires in cases of NPD.

​The therapist needs to be aware of 'narcissistic abuse syndrome' and the concept of gaslighting.
Plans should be in place. ​"What is your approach in course of therapy one needs to decide what to do when one partner refuses to take accountability or lacks empathy. ​Regarding Strategy and Safety, ​therapist should prefer individual sessions first. (In many toxic dynamics, individual sessions are safer because the narcissistic spouse may use what is said in joint therapy as "ammunition" later).

Difficult stages in therapy:
1)​"How do you handle 'blame-shifting' during a session?"
​2) How to develop a safety plan or a 'boundary-setting' strategy if the behavior escalates?
3) ​The professional needs to visualize how the roles in your home have shifted. This can therapist understand the couple's situation more clearly.
The partner needs to ​journal specific incidents: Write down 3-4 recent examples of "gaslighting" or "blame-shifting." This prevents clients from being overwhelmed or "blanking" during the session.
​The partner also needs to clearly define his/ her if your goal is to save the marriage through intensive therapy or to find a safe exit strategy while protecting your mental health.
