Warum wirkt Psychotherapie bei manchen Menschen kaum oder gar nicht?

Es geht um die richtige Dosis.

Psychotherapie ist ein Lernprozess.
Neue Gefühle wahrnehmen, neues Verhalten einüben – das braucht neue synaptische Verbindungen im Gehirn.

Mit einer Stunde pro Woche, ähnlich wie beim Sprachenlernen, entsteht dieser Effekt oft nicht.

Gerade bei Depressionen ist die Lernfähigkeit des Gehirns eingeschränkt. Deshalb muss Psychotherapie zu Beginn intensiv sein, auch wenn das in der Praxis selten umgesetzt wird.

Studien zeigen das deutlich, selbst wenn es in Leitlinien kaum abgebildet ist.

Wird zusätzlich die Neuroplastizität durch Ketamininfusionen oder transkranielle Magnetstimulation verbessert, lernt das Gehirn noch schneller.

So lässt sich Psychotherapie wesentlich intensiver und wirksamer gestalten.



Why does psychotherapy work poorly or not at all for some people?

It’s a matter of dose. Psychotherapy is a learning process – noticing new emotions, changing behaviour, forming new connections in the brain.

With one hour per week, this often isn’t enough, especially when depression further limits learning capacity.

Psychotherapy needs to be more intensive at the beginning. Many studies show this, even if it rarely appears in treatment guidelines.

When ketamine infusions or transcranial magnetic stimulation are added to increase neuroplasticity, the brain learns faster. This makes psychotherapy much more effective.

„Macht eine Suchttherapie mit Ketamin abhängig von Ketamin?“

Die Antwort lautet: Nein!

In der Suchtbehandlung arbeiten wir mit einer Minidosis, langsam über etwa 40 Minuten in die Vene gegeben – nicht mit den hundertfach höheren Mengen, die in der Drogenszene konsumiert werden.

Die Therapie erfolgt immer unter ärztlicher und psychotherapeutischer Begleitung. Diese kontrollierte Anwendung öffnet ein therapeutisches Zeitfenster, in dem sich das Suchtgedächtnis überschreiben lässt – kombiniert mit Psychotherapie, rTMS, Hypnose oder Verhaltenstherapie.

Die Behandlung von Patienten mit schädigendem oder abhängigem Alkoholgebrauch, Kokainsucht oder anderen Abhängigkeiten erfolgt bei uns diskret, in Einzelbehandlung, mit hoher Intensität und multimodal. Dabei werden modernste Erkenntnisse der Suchtforschung angewandt. Bereits nach kurzer Zeit lassen sich stabile Ergebnisse erreichen.

Das unterscheidet unser Therapiekonzept deutlich von konventionellen Suchtkliniken.

Treatment of Depression – using neuroplasticity to achieve lasting results.

For ketamine therapy to deliver sustainable improvements in depression, it must always be combined with psychotherapy.

At the International Conference on Ketamine, Psychotherapy & Brainstimulation 2025 in Frankfurt, Prof. Dr. Martin Walter from the University of Jena highlighted how crucial the timing between ketamine infusion and psychotherapy is.

Research shows that the state of enhanced neuroplasticity induced by ketamine is temporary –
by the second day after infusion, its effect already starts to decline.

This has been confirmed in several large studies.
To make full use of this short therapeutic window,
Dr. Silvana de Lorenzo (Palma) and Alex Ni (Toronto) presented evidence that transcranial magnetic stimulation (TMS) can further support and extend this phase of neuroplasticity.

Rewriting the treatment story of anorexia nervosa.
Among all psychiatric disorders, anorexia nervosa has one of the highest mortality rates.

It can be treated psychotherapeutically — especially when the patient’s family is actively involved.
And yet, many cases still end in chronic illness or fatal outcomes.

At the International Conference on Ketamine, Psychotherapy & Brainstimulation 2025 in Frankfurt, Dr. Amir Reza Bahadori from the University of Tehran presented recent scientific evidence suggesting that brain-stimulation techniques may improve treatment outcomes in anorexia nervosa.

A recent systematic review and meta-analysis indicates that repetitive transcranial magnetic stimulation (rTMS) — a method that uses pulsed magnetic fields to activate specific brain regions — can support recovery by enhancing neuroplasticity, making the brain more receptive to psychotherapy and behavioral change.

Clinical findings show reductions in comorbid symptoms such as depression and compulsions, and in several studies also improvements in weight and BMI.

Preliminary observations point to potential synergies with ketamine-assisted therapy, although larger studies are still pending.

The results observed in our clinical practice confirm the promise of this combined approach — and larger collaborative studies are already being prepared.

Alex Ni (Toronto) demonstrated live EEG recordings showing what happens in the brain during meditation and during a ketamine infusion.

The data revealed signs of increased neuroplasticity — moments when the brain becomes more flexible, more receptive, and psychotherapy can reach deeper.

When light becomes medicine.

At this year’s International Conference on Ketamine, Psychotherapy & Brainstimulation in Frankfurt, Thomas Feiner, BCIA BCN QEEG-D (Munich) shared fascinating insights on how Photobiomodulation (PBM) — the use of low-dose red or near-infrared light — can stimulate neuroplasticity and bridge the gap between neuroscience and psychotherapy.

By combining PBM with Neurofeedback, tDCS, and ketamine-assisted therapy, new pathways open up for patients with affective disorders, cognitive decline, and treatment-resistant depression.

Es gibt viele Mythen und Missverständnisse rund um Ketamin, da es auch in der Drogenszene verwendet wird.

Allerdings in deutlich höheren Dosierungen und nicht als kontrollierte Infusion, sondern als Einzeldosis.

Beim therapeutischen Einsatz werden sehr geringe Dosen verabreicht – medizinisch begleitet und ohne Abhängigkeitspotenzial.

Ketamin kann in bestimmten Fällen helfen bei Depression, Angststörungen, Zwang, PTBS, chronischen Schmerzen und Sucht.

Nach bereits 70 Minuten ist der Wirkstoff vollständig im Körper abgebaut.

There are many myths and misconceptions about ketamine because it is also used recreationally.

However, this happens in much higher doses and not as a controlled infusion, but as a single dose.

In therapeutic use, only very small amounts are administered – medically supervised and without any risk of dependence.

Ketamine can help in cases of depression, anxiety disorders, OCD, PTSD, chronic pain, and addiction.
