⭐️ A 63-year-old patient with multiple chronic medical conditions was admitted to the hospital for nonpurulent cellulitis at the base of the neck. The diagnostic workup revealed methicillin-resistant Staphylococcus aureus (MRSA) bacteremia and a urinary tract infection. Chest radiography and transthoracic echocardiography were unremarkable.
After five days of appropriate culture-guided intravenous antibiotics, the patient's inflammatory markers remained elevated, and blood cultures continued to grow MRSA. Skin examination revealed lesions on the soles of the feet (see photo).

⭐️A 56-year-old female presented with a four-day history of breathlessness, cough and a high fever.
She gave a long history of indigestion and intermittent dysphagia to solids and liquids. She would often wake at night with episodes of coughing and spluttering. There was no history of weight loss. On examination, she had a temperature of 39°C, resp rate of 30/min and a HR of 102. On auscultation of the lung fields there were coarse basal crackles.

⭐️A 45 year old man, known to inject drugs, and currently living in homeless hostels, presents to the Emergency Department complaining of malaise, fever, rigors and arthralgia. On examination:Pt looks feverish, tired, Respiratory: normal breath sound, no crepitation, On auscultation of heart: he has a new pansystolic murmur heard loudest over the left sternal edge.

Laboratory investigations: WBC 15400, CRP 48, RF Is positive.

⭐️ A 77-year-old woman with achalasia was evaluated in a gastroenterology clinic for intermittent dysphagia to solids, regurgitation of liquids and solids, and recurrent esophageal yeast infections over the previous 11 years, as well as a 7.7-kg (17-lb) weight loss over the previous 3 years.

⭐️Viral Pericarditis⭐️

A 44-year-old man sought consultation due to symptoms including palpitations, severe exhaustion, nocturnal coughing, and dyspnea that had been present for two weeks. Before experiencing a fever, chills, and rapidly progressing shortness of breath, as well as trouble breathing and an acute cough when in a supine posture, he was feeling fine.
⭐️In the mean time, he took acetaminophen with vitamins C and D without consulting anyone. He appeared with his spouse 🚺 with a look of exhaustion 😫. He was leaning forward in the chair, slightly relieved from his tension, tachycardia, and polypnea, not feverish, and saturating 97% of the surrounding air. Hepatojugular reflux, hepatomegaly, and turgidity of the jugular veins. Auscultation reveals pericardial friction. The ECG detects a PR segment depression and tachycardia.
