Sub trochanteric open Fracture

A 22 year-old male presents after RTA. He complains chiefly of right hip pain and bleeding of it's wound.

It's fixed by external fixation by emergency orthopedic team of NRH .

Supracondylar fractures most often result from forcible hyperextension at the elbow. The periosteum acts as a hinge, but rotational forces may allow the fracture to tear the periosteum. This can result in gross displacement, button-holing of the brachialis muscle, and place the neurovascular structures at risk. As with most displaced fractures and dislocations, the first step is obtaining a reduction and re-evaluating pulses, as arteries may be kinked or in spasm. Closed reduction and percutaneous pinning (CRPP) will likely restore pulses in this patient. Serial vascular exams are warranted in the post-operative period. If pulses do not return in a timely manner and the hand is pale, open exploration with possible vascular repair and possible fasciotomies is indicated.

Open Both Bone Fracture in 9M

HPI: A 9-year-old male fell over the handlebars of his bicycle and landed on his right wrist. He noted immediate pain and deformity to the wrist. Denies any numbness/tingling to his fingers.

PSH: Previous left BBFA fracture treated with ORIF (1 year prior). Otherwise healthy.

PE: A focused physical examination of the right upper extremity reveals a small 3mm open wound over the ulnar aspect of the forearm in continuity with the fracture site. There is a gross apex ulnar deformity of the forearm. The wrist ROM and strength were limited secondary to pain. He is neurovascularly intact.
