One of the most devastating things any parent can experience is the loss of a child. Whether in early or late pregnancy, during childbirth or later in life, it can be an overwhelming and heartbreaking experience. The month of October is Pregnancy and Infant Loss Awareness Month and Oct 15 is Pregnancy and Infant Loss Remembrance Day. During this time, we take time to remember those babies we have lost and the parents and families who remain to mourn. As we go through this month, we consider why these events happen, so that we can work to prevent them happening again.

Vaginitis or inflammation of the va**na is not always caused by infections. A common cause is a retained foreign body, usually a tampon. A forgotten tampon or condom is usually associated with a discolored, sometimes blood-stained discharge, which is very smelly. Treatment involves removal of the foreign body. Although there are usually no serious complications, if neglected for long, a foreign body may cause infections which may spread to other parts of the pelvis. It is important to ensure that all tampons and condoms are removed after use.

Trichomoniasis is an infection of the va**na and sometimes the urinary tract with an organism called Trichomonas va**nalis. It is a sexually transmitted infection that causes a heavy, yellow or green, foul-smelling discharge. It may cause soreness and itching of the v***a, painful urination, and pelvic pain. Males may develop a urinary tract infection. Trichomoniasis may cause chronic pelvic inflammation if untreated and may also be associated with preterm labour. If trichomoniasis is diagnosed, tests must be done for other sexually transmitted infections and sexual partners should be treated. Treatment is usually with oral antibiotics.

The va**na contains many different types of bacteria. Bacterial vaginosis or BV occurs when there is a change in the numbers and types of bacteria in the va**na. BV is not considered a sexually transmitted infection but is more common in sexually active women. BV causes a thin, slightly discolored discharge with a fishy odour and may be associated with burning and mild v***ar irritation. It is treated with oral or va**nal antibiotics.

Candidiasis, commonly called “yeast infection”, is one of the most common causes of inflammation of the v***a and va**na. Candida is normally present in the va**na and an overgrowth of this organism causes yeast infections. Candidiasis is not considered a sexually transmitted infection. Use of oral contraceptive pills, pregnancy, antibiotics and problems with the immune system can be associated with an overgrowth of Candida. This results in itching and soreness of the v***a and a thick discharge resembling cottage cheese. Candidiasis may be treated with oral medication or va**nal creams or tablets.

The tonsils and adenoids are part of a ring of tissues that are involved in the detection of foreign materials entering the body and the formation of antibodies against them. Sometimes, due to inflammation, infection, or obstruction, their removal is necessary. Here at Rovier Surgical Suites, we will usually remove tonsils and adenoids as a same-day or overnight surgery, and the child goes back to normal living in one to two weeks. Tonsil and adenoid disease can lead to blockage of the tube connecting the ear to the back of the nose. This can lead to fluid accumulation behind the eardrum with hearing loss. Tubes placed in the eardrum can drain this fluid with immediate improvement in hearing. This is usually done as an outpatient procedure with the child in and out in a few hours. RSS is the only facility on the island completely equipped to carry out this procedure. Usually, the tubes will remain for 6 to 18 months and then will be extruded spontaneously. Sometimes They may have to be replaced if the fluid recurs. In that case, special long-stay tubes may need to be used for longer placement.

Sleep apnea can often accompany snoring in children. This will lead to daytime sleepiness, difficulty with concentration, obesity, and ADHD. Clearing up associated infections and using steroid nasal drops can help for a while, but the problem is often due to adenoid and tonsil enlargement. In these cases, it is easily cured by removal of the tonsils and adenoids, and the response is often quite dramatic, with the child sleeping quietly from the first night. Snoring occurs when there is a blockage in the upper airway. In children, this blockage is most commonly due to enlargement of the tonsils and adenoids. Snoring in children can cause daytime sleepiness, difficulty concentrating, hearing loss, or even heart failure.
