Classification of Burning Mouth Syndrome into Primary BMS and Secondary BMS.

📍Primary BMS: The most recent investigations suggest the hypothesis that the cause of Primary BMS could be a Primary Neuropathic Dysfunction, that is, an alteration of the thermal and pain receptors of the tongue, with sensory involvement of the small fibers of the trigeminal nerve. While other studies suggest the hypothesis of a condition of the taste system, mediated by the chorda tympani nerve.

📍Secondary BMS: Common medical conditions that could cause BMS include:
* Metabolic Disorders (diabetes)
* Allergies to dental products or certain foods, especially acid foods.
* Dry mouth, caused by Sjögren Syndrome or treatments of radiation therapy.
* Certain medicines such as psychiatric drugs, drugs to reduce blood pressure.
* Hormonal changes (associated with menopause)
* Nutritional deficiencies (vitamin B B12 or iron).
* Infection in the mouth, such as a yeast infection, Helicobacter pylori, ge.
* Gastroesophageal acid reflux.
* Parafunctional Habits (bruxism, tongue trust, clenching teeth, etc).

The specialist in Oral Pathology and Oral Medicine mainly, or the Clinical Physician or a Dermatologist Doctor, must investigate these aspects, through an exhaustive evaluation, with a laboratory test. If these causes are treated and the burning in the mouth disappears, its origin was associated with these causes. But if after investigating and treating these causes, the burning mouth still persists, you can say it's downright BMS.

Clasificación del Síndrome de Ardor Bucal (SAB) en SAB Primario y SAB Secundario.

📍SAB Primario: Las investigaciones más recientes sugieren la hipótesis de que la causa del SAB Primario podría ser una Disfunción Neuropática Primaria, es decir, una alteración de los receptores térmicos y del dolor de la lengua, con afectación sensitiva de las pequeñas fibras del Nervio Trigémino. Mientras que otros estudios sugieren la hipótesis de una afectación del sistema gustativo, mediada por el Nervio Cuerda del Tímpano.

📍SAB Secundario: las condiciones médicas comunes que podrían causar SAB incluyen:
* Trastornos metabólicos (diabetes)
* Alergias a productos dentales o ciertos alimentos, especialmente alimentos ácidos.
* Boca seca, causada por el Síndrome de Sjögren o tratamientos de radioterapia.
* Ciertos medicamentos como los psiquiátricos, fármacos para reducir la presión arterial.
* Cambios hormonales (asociados a la menopausia)
* Deficiencias nutricionales (vitamina B o hierro).
* Infección en la boca, como candidiasis o Helicobacter Pylori, por ejemplo.
* Reflujo ácido gastroesofágico.
* Hábitos Parafuncionales (el bruxismo o el apriete dentario, entre muchos otros).

El especialista en Patología Oral y/o Medicina Oral principalmente ó el Médico Clínico, ó el Dermatólogo, son los Profesionales más apropiados para investigar estos aspectos, mediante una exhaustiva evaluación y pruebas de laboratorio.
Si estas causas son tratadas y el ardor en la boca desaparece, su origen claramente estuvo asociado a estas causas. Pero, por el contrario, si después de investigar y tratar estas causas, el ardor en la boca aún persiste, se podría decir que francamente estamos frente a un paciente con diagnóstico de Síndrome de Ardor Bucal.

If you are a BMS patient, it is very important to be aware of the difference between "hot" and "very hot" foods or liquids when eating. From a nutritional point of view, a food is considered to be "very hot" when its temperature exceeds 50°. The ingestion of drinks or food at these temperatures can trigger inflammation or redness of the oral mucosa, especially in the anterior palate, incisive papilla, tip of the tongue, anterior third and lateral edges of the tongue. These organs and structures are essential for chewing, swallowing food and speech.
In addition, ingesting liquids and foods at excessive temperatures can cause an alteration of the taste buds (specialized mucosa), intensify the burning sensation in the mouth and alter taste and orofacial sensitivity for several days.
In summary, patients with BMS present inflammation and irritation in the nerve endings of the tongue, which are located under the oral mucosa.
If you eat food with temperatures above 50°, it could be able to produce more irritation and a decrease in the tactile function of the tongue.
In this situation, you could perceive a decrease in oral sensitivity, which will lead you to make greater muscular efforts when chewing and swallowing food, increasing your symptoms of burning in your mouth.

A classification of BMS was divided in 3 types (Lamey PJ.,1996), according to the time of appearance of burning symptoms.

TYPE 1: They are patients who experience burning in the middle of the morning which increases in intensity every day, towards the night.

TYPE 2: These are patients who wake up with a burning sensation in the mouth which continues throughout the day, every day.

TYPE 3: In these patients the burning occurs intermittently. Some days it is present and other days it is not.

The characteristic symptomatic triad of Burning Mouth Syndrome (BMS) is made up of:

- BURNING SENSATION: in one or more oral soft tissues such us tongue, lips, inner face of the cheeks and palate, among others.

-XEROSTOMIA: subjective sensation of dry mouth.

- DYSGEUSIA: alteration in the taste perception.

It is very common for patients suffering from Burning Mouth Syndrome to manifest pain or a burning sensation, especially on the tip of the tongue, the labial mucosa, buccal mucosa and palate.

In addition, they also refer to an alteration in the perception of taste described as a bitter, metallic and sometimes salty taste.

Symptoms of dry mouth are also frequent in BMS patients, which causes them great difficulties in performing the orofacial functions of chewing, swallowing, phonation and speech.

Burning Mouth Syndrome (BMS) is a clinical entity characterized by a burning sensation in different sites of the oral mucosa.
It has also been named oral dysaesthesia, glossodynia and stomatodynia, among others.
Its etiology is multifactorial and tends to affect more women than men.
