HBV-positive and HIV-positive organs in transplantation: A clinical guide for the hepatologist

Transplantation from donors positive for HBV and HIV has increased in many countries. The intentional transmission of a virus has practical, ethical, and clinical implications. In this review, Russo and colleagues summarise the current research, focusing on grafts from donors positive for the HBV surface antigen, antibodies against the HBV core antigen, and HIV, to help hepatologists and physicians interested in transplantation to select the best antiviral and/or prophylactic regimens for after transplantation.

A placebo-controlled randomised trial of budesonide for primary biliary cholangitis following an insufficient response to UDCA. Read open access at bit.ly/2UomNZa

In patients with primary biliary cholangitis (PBC) the utility of budesonide is unresolved. In a placebo-controlled, double-blind trial, this study evaluated the added-value of budesonide in those with PBC and ongoing risk of progressive disease despite ursodeoxycholic acid (UDCA). Budesonide did not associate with improvement in liver histology; improvements in biochemical markers of disease activity were demonstrated in secondary analyses.

Herein, the authors review the roles of multiple cell types that are involved in inflammation in ASH and highlight the concept that inflammation also plays an important role in promoting liver repair and controlling bacterial infection.
