Functional Hypertrophy for ACL Rehab

The quads are the first line of defence for the knees when it comes to absorbing load and creating stability around the joint. Developing high levels of movement ability, mobility, hamstring calf and glute strength, force absorption, and sport specific loading are all necessary…but the first priority needs to be getting the quads strong, big, and functional at all joint angles.

This finding from Green et al. certainly has multiple factors that make up the full picture!
1. Often grafts are taken from the hamstring for surgical ACL repairs which will leave the hamstring potentially more vulnerable to strain when reintroducing high speed running or competitive performance. This accounts for some but not all cases.
2. We know hamstring and soleus are the muscles around the posterior knee that do the most to mitigate anterior sheer forces (the same forces that the ACL resists against in the knee). Returning to sport, it is plausible that these muscles will work harder to perform this task so that a vulnerable ACL won’t have to. This explains the increased risk of hamstring strain if there has been a previous calf strain and it has to pick up even more of the slack!
3. The last factor may be general deconditioning after an ACL injury predisposing athletes to numerous types of injury when they return to sport without adequate preparation. Too often the level of rehab and preparation does not meet the standards needed for a return to high performance.
