🩸 Subhyaloid Hemorrhage

A subhyaloid hemorrhage is a collection of blood trapped between the retina and the posterior hyaloid face of the vitreous body inside the eye.

Appears as a boat- or D-shaped pool of blood, often overlying the macula — the central part of the retina responsible for sharp vision.

This condition leads to sudden, painless loss of vision, especially if the hemorrhage obscures the macula.

A patient presented to our clinic with complaints of cloudy vision in the left eye for the past one week. On examination, visual acuity in the affected eye was 6/9, and initial OCT scan appeared within normal limits. However, upon thorough evaluation using indirect ophthalmoscopy in various gaze positions, a superior retinal detachment was identified. This finding was later confirmed by OCT imaging with altered gaze fixation.

Importantly, the central fovea remained attached, classifying this as a fovea-sparing retinal detachment. In such cases, where early detection and precise localization are key, pneumatic retinopexy is considered a suitable treatment option.

Pneumatic Retinopexy is a minimally invasive procedure that involves injecting a small gas bubble into the eye. The gas bubble rises and presses against the detached retina, allowing it to reattach naturally. The patient is then positioned in a specific way to help the bubble remain in contact with the affected area. This technique offers faster recovery and avoids the need for more extensive surgery in selected causes.
