Registered Nurse – Community Care Coordinator (Part Time)

We are seeking a compassionate Registered Nurse to join our community care team in a part-time role. This position is a combination of office based and community-based work. The primary focus of the role is overseeing client services, reviewing clinical reports completed by RNs and ENs, identifying any required follow up actions, and supporting quality and compliance across our community care services. Training and support will be provided.

About You:
• Current AHPRA RN registration
• Strong clinical judgement and communication skills
• Organised and able to manage oversight of multiple client services
• Compassionate and team focused
• Driver’s licence, reliable vehicle, Police Check, First Aid/CPR, NDIS Clearance & WWCC

Community nursing, care coordination, NDIS, aged care, or disability experience is beneficial but not essential.
