GP REFERRAL OPTIONS YOU SHOULD KNOW ABOUT

🟩 GP Chronic Condition Management Plan (GPCCMP):
Set up by your GP, a GPCCMP supports people living with chronic conditions to access coordinated, personalised care. The plan focuses on your individual needs and provides access to Medicare-subsidised allied health services, such as physiotherapy.
A GPCCMP can include up to 5 allied health sessions per calendar year, shared across services agreed on with your GP.
Referrals are valid for 18 months.

🟧 Sonder:
Sonder’s Integrated Primary Care program supports people with chronic medical conditions to better manage their health and wellbeing. Your GP and a Sonder consultant will work with you to develop a personalised care plan, which may include access to multiple allied health services - like physiotherapy - at no cost. Sonder is available once a GPCCMP has been used, or, has been allocated to a different health care professional.
Referrals are valid for 12 months.

This week we celebrate 2 years of business. Our second year has been big and exciting where we:
- Opened to 5 days a week

CHRISTMAS/NYE CLOSURE

As Christmas and NYE approaches, our team is looking forward to a well deserved break. Yorke Peninsula Physiotherapy will be closed from 4pm on the 19th of December 2025 and reopening at 8am on the 5th of January 2026.

NEW CLASS TIME

Our classes are getting more popular so we are now offering a Gym Rehab Class on Tuesday's at 9am. These classes will start next week, the 4th of November.

Gym rehab classes are:
- 45 minutes
- Have a maximum of 6 participants
- Private Health rebatable
- Require an initial appointment to design a tailored exercise program

Get in touch if you'd like to move better and have supervision while you exercise!

WELCOME ISABELLE!

If you have been to the clinic this week, you might have had the pleasure of meeting our newest team member Isabelle! She will be treating every Monday, Wednesday and Friday.

Isabelle graduated with a Bachelor of Physiotherapy (Honours) from the University of South Australia in 2024. That same year, she completed her Clinical Mat and Equipment Pilates training through the Australian Physiotherapy and Pilates Institute. In 2025, Isabelle went on to become a certified GLA:D hip and knee physiotherapist.

Isabelle has a passion for helping people of all ages reconnect with positive moment, return to optimal function and enjoy an improved quality of life. She has a keen interest in sporting injuries and rehabilitating players back to competition, orthopaedic rehabilitation and general musculoskeletal issues. She values the role of movement complemented by hands on treatment.

Growing up in the small town of Millicent in the South East of South Australia, Isabelle is excited to bring her skills and knowledge back to a regional community. Her goal is to provide care and guidance that allows patients to feel empowered and supported throughout their rehabilitation journey.

Outside of work, Isabelle enjoys playing netball, walking along the beach, spending time with friends and family and cooking up new recipes in the kitchen.
