Chronic pain is very common in the general population. Hyperbaric oxygen therapy has been studied in various chronic pain syndromes, including fibromyalgia syndrome, complex regional pain syndrome, myofascial pain syndrome, migraine and cluster headaches. Although the results of these studies are promising, routine management of these chronic pain syndromes with hyperbaric oxygen therapy cannot be justified at this time. However, hyperbaric oxygen therapy may be used in selected patients unresponsive to other treatments, where facilities are available, after carefully weighing potential benefits against the risks of treatment. Further large-scale, good-quality, randomized, controlled studies are required to clearly demonstrate efficiency, cost–effectiveness, optimal treatment protocol and safety of hyperbaric oxygen therapy in the management of chronic pain.

What role could Hyperbaric Oxygen Therapy (HBOT) play in supporting patients with neurodegenerative disease?

There are certain common denominators that virtually all the diseases in this category share.

Some of these include mitochondrial dysfunction, chronic inflammation, chronic hypoxia in the regions of the brain that are being affected, and chronic inflammation within the central nervous system itself.

Now, what is it that we know about HBOT?

We know that HBOT will deliver oxygen to all of the working cells and tissues in our body.

So, if we have an area of chronic hypoxia (like we do in most neurodegenerative diseases) increasing the oxygen level will act as a fuel to help support that hypoxic cell and tissue.

We also know that HBOT helps to reduce the cytokine response and reduce the inflammatory response systemically throughout the whole body.

HBOT delivers oxygen to the working cells to increase cellular energy production and will reduce oxidative stress in the area.

We also know that HBOT will stimulate improved synapse connection with nearby neurons, improving neuroplasticity within the brain.

What might some different protocols look like?

Our protocols would range from 1.3 to 1.55, the overwhelming majority getting somewhere between 1.3 and 1.5 ATA.

A very standard program for us would be 60 to 90 minutes per session, 3 to 5 sessions per week for a minimum of three months.

Hyperbaric oxygen therapy at 1.3 atm at room temperature is just as effective as 100% oxygen at 2.4 Atm as evidenced by research

Breathing regular air under hyperbaric oxygen conditions of 1.3 Atm leads to more than 50% elevation in tissue oxygenation. There are many case reports illustrating significant effects due to small increases in air pressure, including effects on the brain (2,3,4,5). Moreover, even a slight increase in partial pressure, such as to 1.05 Atm at altitude 402 m below sea level (the Dead Sea) can lead to noticeable physiological effects (6-10). Since 50% elevation in tissue oxygen can have significant physiological effects, treatment with room air at 1.3Atm is not an "ineffectual treatment"..(1)

A recent randomized, controlled trial on mTBI patients by Wolf et al (11), used room air at 1.3 Atm as sham control for treatment with 100% oxygen at 2.4 Atm. Both groups revealed significant improvements in cognitive symptoms and in the measure of post traumatic stress disorder (PTSD). WE find these results very important: they actually demonstrate that the significantly less expensive and logistically simpler treatment of mTBI patients with mild HBNO2 (mild hyperbaric pressure of 1.3Atm and regular air) can lead to meaningful improvements. Interpretation is based on previous studies demonstrating that mild HBNO2 conditons can be effectual treatments (1).

For the first time, convincing results based on a crossover study, demonstrating that HBOT can induce neuroplasticity and significant brain function improvements in mild TBI patients with prolonged Post-Concussion-Syndrome at late chronic stage, years after injury. HBOT can be effective in treating other brain impairments, like easing PTSD symptoms or reparing radiation damage. It is also reasonable to expect that HBOT can help slow down or even reverse metabolic disorders associated with neurodegenerative diseases (1).

Breathing 100% oxygen at 2.4 ATA generate very high oxygen levels in tissues, which can cause an inhibitory effect or even focal toxicity, it is conceivable that HBOT using 2.4 ATA can be less effective than 1.3 ATA or other lower levels of pressure (5).

Dani Hewton is a qualified teacher and wellness coach. Rob was told he was unlikely to ever work again and was taking large, debilitating doses of strong painkillers, anti-inflammatory, nerve blockers and steroids for over 2 years. As a result of the relief provided by the infrared cocoon and sauna, Rob has managed to stop taking most of his medication and is now functioning better, moving better, thinking better and is now even able to work again.

Ongoing chronic pain impacts everyone in the sufferer’s life. The stress and frustration can seriously damage relationships and the self-worth and security of the sufferers loved ones.
