WHAT: a dermatophyte infection of the nail bed that causes destruction of the nail..

WHO: anyone can be affected though it seems to be more common in people of the age of 50
••
SYMPTOMS: discolouration of the nail. Can be white, yellow, orange, even brown. Nails can become thick and crumbly and may start to lift
••
TREATMENT: first you need to be assessed to make sure it’s definitely a fungal infection. The nail will be burred down and advice will be given on the best course of treatment. Options can include: anti- fungal paints, tablets or laser. Accurate diagnosis is required. The fungus can spread if left untreated so it’s advisable to seek advice from a podiatrist ASAP

A very useful tool in diagnosing boney conditions. They help us to check joint spaces, alignment of joints and bones (if taken weight bearing), boney lesions, different forms of arthritis, fractures and breaks..

They do not tell us anything about the integrity of soft tissue structures like ligaments..

I don't think there is a group of health professionals that watch people walk more than us. Yep, we are secretly assessing you as you walk through Woolworths. Of all the things we learn at uni, biomechanical assessment is the most complex and the subject guaranteed to cause the most stress and confusion.

There are 33 joints in each foot, not to mention the ankle, knee and hip joints. These joints move (mostly) in 3 different directions and during gait they are in a constant state of movement. And guess what? Every single person is different. Then you lay the muscles, tendons and ligaments over the top and what you are left with is an incredible complex assessment task. You want to know why we spend 4 years at uni? Well that's why (cutting toenails is actually only a tiny part of our training)

Gait or walking assessments help us to understand how the body functions. Assessment while lying down is only part of the picture. You need to see what happens functionally. A perfect example is ankle joint range of motion. So often when a patient is sitting in the chair, they have 'normal' movement at their ankle, but once on their feet, this movement greatly reduces. Something has changed and it is our job to work out why. If we only assessed a patient non-weight bearing, we would never even know there was a potential issue.

A note on fancy equipment in shoe stores. These pressure plates and video recording give some information, yes. But they don't provide all the information. To get a full picture, you need to be assessed by a podiatrist, or another health professional with a large amount of experience or additional training in gait analysis.

WHAT: a very common skin lesion caused by a virus. Once in your system, warts can reoccur at any time

WHY: they occur when unresilient skin is exposed to the virus. Macerated, wet and rubbery skin like what happens when your skin is exposed to water for a long time, is a perfect time for the virus to pe*****te. That’s why pools and communal showers are often blamed..
••
PREVENTION: as the saying goes ‘prevention is better than a cure’ and this is really true in the case of warts. Always wear thongs in communal showers and around pool decks..

TREATMENT: treatments can be slow. Acid paints are the best option for the sole of the foot, liquid nitrogen (freezing) doesn’t generally work and it’s painful. Newer techniques include laser therapy as well as puncture therapy which works well for very large lesions. As a last resort surgery may be required..

WHAT: A common fungal infection of the skin. It is extremely common on the feet.

WHERE: Anywhere on the skin but commonly between the toes as there is poor airflow and the area is prone to becoming moist. Symptoms include redness and itchiness in the area. There may be small raised lumps or very flakey white skin. Itchiness is almost always involved.
••
AFFECTS: Anyone. Thrives in warm, moist skin, sweaty socks and shoes. The fungus that causes tinea is very contagious and can be found in most communal areas such as swimming pools, hotel carpets, bath mats.
••
TREATMENT: involves using an anti-fungal product either in the form of a cream or a spray. Creams should never be used between the toes as creams make the area even more moist. It is really important that the socks and shoes are also treated. WHAT: Ulcers are often caused by excess pressure. In this case, the big toe joint (1st MPJ) can’t move, and you really really need that joint to move. So the clever body has decided to move the next joint more and thereby shunting all this persons weight to this tiny joint which can not stand the pressure. It allows the body to keep moving forward but at a huge cost

TREATMENT: removing or offloading the pressure is the most important step. This is not as easy as it sounds. It usually involves orthotics, and changes of footwear. Once the pressure is removed then the harden skin needs be removed regularly and special round healing dressings applied to allow the ulcer to close up from the bottom. It can take months. Sometimes years, and can sometimes result in amputations.
