For many women, fertility concerns are often focused on the egg. But research is showing there’s more to the story than egg age alone.

As women get older, fertility naturally declines. This is usually explained by changes in the number and quality of eggs. However, scientists are learning that the environment surrounding those eggs – the ovaries and the tissues within them – also plays an important role.

Think of the o***y as a whole ecosystem. It’s made up of support cells, blood vessels, nerves and connective tissue that help eggs grow and mature. When this environment changes with age, it can affect how eggs develop.

The ovaries don’t just store eggs – they also produce hormones that guide eggs through the process that leads to ovulation each month. Eggs develop inside tiny structures called follicles, and the surrounding environment helps protect and support them along the way.

Recent research comparing younger and older ovaries found that, with age, blood flow around follicles can decrease while certain nerve signals increase. Together, these changes may affect the health of the follicles and the eggs they support.

In simple terms, the egg is important, but it needs the right support to thrive.

That’s why looking after your overall health can make a meaningful difference to your fertility. Maintaining a healthy weight, eating a balanced diet, staying active, managing stress, and limiting alcohol and smoking all help support ovarian health and may reduce the impact of reproductive ageing.

Not every egg is ready to become an embryo.

For fertilisation to succeed, an egg must be mature. Only mature eggs have the correct chromosomal makeup to successfully merge with s***m and support normal embryo development.

An egg is classified as mature once it has released a tiny cell called a polar body, which can be seen within the outer shell of the egg. In the first image, the polar body is visible at around the 12 o’clock position.

Immature eggs haven’t completed the necessary stages of development, so a polar body isn’t present – as shown in the second image.

Fertilisation relies on balance. Both s***m and egg must each contribute exactly half of the genetic material. Without egg maturity, fertilisation may not occur – and even if it does, healthy embryo development is unlikely.

The numbers don’t lie – more women are choosing to start families later in life.

As careers, financial security and broader life circumstances – including finding the right partner – take priority, childbirth is increasingly occurring at an older age. In Australia, the average age of women giving birth has now risen to 31.3 years.

Births to women aged 35 years and over continue to increase, while births to women under 25 years have declined significantly. Today, babies are more likely to be born to mothers aged 40+ than to teenagers.

At the same time, fertility treatment and technology have advanced rapidly. Improvements in embryo culture, freezing techniques and genetic testing have led to stronger outcomes, supporting more women to begin or expand their families later in life.
