Hiring experienced Support Coordinators, can be remote, but will need to attend office at least one day per week. $70 per hour starting rate.
Key Responsibilities

Plan Implementation: Assist participants in understanding their NDIS plans and ensure the effective implementation of these plans to achieve their goals.

Service Coordination: Connect participants with appropriate support services, manage and coordinate the delivery of services, and monitor the effectiveness of these supports.

Capacity Building: Help participants build their capacity to navigate the NDIS, make informed decisions, and develop skills for greater independence.

Stakeholder Engagement: Work closely with participants, their families, and various stakeholder to foster collaboration and address any challenges that may arise.

Budget Management: Manage participant budgets, ensuring that services are delivered within the allocated funding and that financial records are maintained accurately.

Reporting and Compliance: Prepare necessary documentation and reports, ensuring compliance with NDIS practice standards and organizational policies.

Required Qualifications and Skills

Interpersonal Skills: Strong communication and interpersonal skills are essential.

Organizational Skills: Ability to manage multiple tasks, maintain accurate records, and ensure timely reporting.

IT Proficiency: Familiarity with IT systems and databases related to NDIS, including PACE and CRM’s and NDIS, is often necessary.

Commitment to Inclusion: A strong commitment to the values of inclusion and respect for the rights of individuals with disabilities.

You must have a insured vehicle, Blue card, Worker screening card and first aid, cpr, your own tablet or laptop.
You must be willing to come into the office and do admin work once a week.
Work Environment
Support Coordinators typically work in community settings, collaborating with various service providers and stakeholders to ensure that participants receive the support they need to achieve their goals. The role may involve travel to meet with participants and service providers in different locations.

Hi Everyone,

At Achieve Disability Supports we are connecting with many services within our local community, throughout Moreton Bay and the greater Brisbane. We would like for you to know these services and opportunities too. We will begin regularly sharing with you about a variety of topics and hope you find this information helpful.

As we head to the end of the financial year, we thought we would start with many of the financial services available for you.

Michelle one of our support workers and the community liaison officer for Achieve, recently completed the Saver Plus program. The five online sessions she attended had great information about all things to do with money and you receive further information after each session. It was a very worthwhile program and is highly recommend for anyone who would be eligible and wants to improve how they use it and relate to it. (See the brochure below for more details).

Saver Plus is a 10-month savings program that provides financial education to help build essential skills for managing money and becoming a regular saver. Upon completion, ANZ will match your savings, up to $500, for education-related costs for you or your children. This program assists eligible participants the opportunity to learn how to save $50.00 per month for 10 months. After the savings period, you will have money to go towards educational expenses for your children studying or expenses if you are studying yourself.
