About the role:
You’ll be supporting NDIS participants with day-to-day living and community activities, including:
🧼 Personal care
🛍️ Shopping and errands
🧹 Light cleaning (kitchen, bathroom, floors)
🚍 Using public transport and community outings
🏥 Attending medical appointments

We’re looking for women who:
✨ Are aged 40+
✨ Speak FLUENT English
✨ Are flexible with days and times
✨ Have experience supporting people with intellectual disabilities
✨ Are caring, patient, and reliable

You’ll need:
✅ NDIS Worker Screening Check
✅ Blue Card
✅ Police Check
🚗 Driver’s Licence (preferred but not essential)

Achieve Disability Supports is seeking an experienced and motivated Support Coordinator to join our team. This role is all about empowering participants to navigate the NDIS, build their capacity, and achieve their goals.
About the role:
As a Support Coordinator, you will:
Work closely with participants, families, and providers to implement NDIS plans.
Connect people with the right services and supports.
Build participant capacity and independence.
Coordinate and monitor services to ensure quality outcomes.
What we’re looking for:
Experience in NDIS Support Coordination or a similar role.
Strong knowledge of the NDIS and person-centred practice.
Excellent communication and organisational skills.
Ability to work independently and within a supportive team.
Current NDIS Worker Screening Check, First Aid/CPR, and valid driver’s licence.

In this role, you will support people with disability to live independently and achieve their goals. Shifts may include personal care, community access, transport, domestic support, and social engagement.

Strong communication and interpersonal skills
