First run in the books for .barber 6 mths following acl reconstruction surgery.

This one means a lot.

She didn’t take the easy road.
A quad tendon graft — one of the toughest grafts to rehab. Slower early progress, more quad inhibition, and a real mental test.

But there’s a reason we choose it…

ACL REHAB Case Study

Last week I started working with .bond_.

He was 6 weeks post surgery (acl reco w/ HS graft + meniscus repair).

Big changes in a week after proper load management of the knee using crutches and decreasing step count. Plenty elevation of address the swelling and heaps of quad activation work.

Big difference after a week of doing the right work in early rehab.

ACL Surgery? Avoid These Early Rehab Mistakes

The first few weeks after ACL surgery aren’t about doing more.
They’re about resetting the knee to build a foundation the rest of the rehab builds on.

Here are 5 mistakes I see all the time in Phase 1:

❌ Pillow under the knee
Feels comfortable, but it encourages the knee to stay bent.
Early priority is getting the knee fully straight.

❌ Not elevating the leg high enough
If the knee lives down by your waist, swelling hangs around.
The leg should be elevated high to help fluid drain.

❌ Forcing range of motion exercises
Cranking the knee into bending when it’s swollen and irritated often backfires.
First goal: calm the knee down.

❌ Too many steps too soon
Early rehab isn’t about hitting step goals.
Too many steps can irritate the knee and slow recovery.
✅ Monitor your step count.

❌ Ditching the crutches too early
Coming off crutches before the quad is firing properly usually leads to limping and poor movement patterns.

One simple sign your quad is ready?

The heel pop.
When the quad contracts properly, the knee locks straight and the heel lifts slightly off the bed.
It’s a quick way to see if your Phase 1 foundations are solid.

Phase 1 goals:
• Quiet knee
• Full extension
• Quad firing
• Heel pop
• Normal walking

Get these right, and the rest of rehab becomes much easier.

Don’t just return to play.
Build a knee that can perform.

RP9 ACL Rehab System.

PHASE 1: The Reset Phase

I use a 9-phase system (RP9) to guide ACL clients from surgery all the way back to sport. But before we talk strength, jumping or cutting… we need to reset the knee first.

After ACL surgery the knee is irritated, swollen and the quad muscle basically goes offline.

If you skip this phase and rush ahead, the knee usually fights you the whole rehab process.

So Phase 1 is about CARE.

C – Calm the knee
Reduce swelling and irritation so the joint can actually move and accept load.

A – Achieve full extension
Getting the knee completely straight early is critical for normal walking and long-term knee health.

R – Reignite the quad
The quad shuts down after surgery. We need to wake it back up so it can support the knee again.

E – Establish normal walking
Before strength training or running, we need a smooth, confident walking pattern.

This phase might sound simple… but it sets the foundation for everything that follows.

If the knee is quiet, straight, the quad is firing and you’re walking normally…
you’re ready to actually start building a knee that can perform again.

What I check before clearing ACL athletes to run.

After ACL surgery, most rehab focuses on strength.

But running requires reactive strength — the ability to produce force quickly and rebound off the ground.

Low-level plyometrics like skipping help rebuild the spring before returning to running.”

7 mths of rehab with after acl rupture and reconstruction surgery.

For a quad tendon graft his quad strength is excellent for this time point. His non operated leg is hitting a peak quad force of 888N and his operated leg is getting 739N. Only a 17% quad deficit at this stage is very rare.

When returning to a field sport these sessions are vital.

This is phase 6 of our 9 phase RP ACL system.

The goal is not just to return to playing but returning at a level where you have the confidence in your knee to perform and compete at your highest level.
