Another example of a Composite (Bonding) restoration to close a diastema. Again, something I would normally do if, for some reason, orthodontic treatment is not an option. I feel it’s a technique that definitely has its benefits, but that also needs maintenance in the long run.
In a society in which everyone wants everything done straight away, these kind of procedures are getting more and more importance.

“Composite Bonding” is a fashionable term to describe a composite (white) restoration utilised to close an unsightly space, usually found on the front teeth. The space between the upper central incisors, is called a Diastema, and although in 90% of the cases the space can be closed with a filling, the limit to what can be achieved, is dictated by the Golden ratios, or by what the patient considers acceptable aesthetically. In other words, most of the gaps can be closed, but nobody wants to walk around with a tooth that looks too big and/or out of proportion for the teeth next to it. Everything has to maintain proportions with in the natural range, in order to look aesthetically acceptable.

Tear Troughs, before and straight after shots with Dermal Fillers. This is a difficult procedure and a very delicate area to work on, and carries the risks of very severe and debilitating complications. It is an area that can be improved indirectly by treating the peri-orbital structures (temporal, zygomatic and maxillary processes), although in particular cases that is just not enough, or maybe it’s not needed. In this particular case I placed 0.5ml of Volbella per side, utilising a cannula to minimise trauma. I always advice to place not more than 0.5ml per side per visit and, in case, place another 0.5ml 2 weeks after the initial treatment. Also, always place the filler down to bone, to avoid the occurrence of the Tyndall effect. Do not, I repeat, do not carry out this procedure unless you are properly trained, you have hyaluronidase ready for use, and you’re in touch with an ophthalmology emergency department in your local area.

Treatment of gummy smile with Botox. This was a challenging case that I’m glad to share with you thanks to this lovely patient’s approval. The challenge here was that the occlusal plane is not on the horizontal, following the mid pupillary line.
So ideally, the treatment should consist of orthodontic treatment to rearrange the bite (and that would probably already improve the situation), followed by a ripositioning of the lip with Botox (or surgically). The treatment options were discussed with the patient, but because she didn’t want to embark on a course of orthodontic treatment and surgery, we managed to achieve an acceptable result with the use of Botox alone.
In a situation like this one, we decided to drop the upper lip so that it would be in parallel with the zenith of the teeth, and not with the mid pupillary line. The difficulty here was to drop the right side slightly more than the left side. My only advice here is to start a smaller dose than we would normally administer, and then review at two weeks, to see the result and possibly do a little correction.
It’s always better (and less risky), to start with less, and add on at a later stage if you need to.
In this particular case, after the first treatment, the patient was happy with the result and didn’t want to add more to drop the lip a little further 🙂

Hands Rejuvenation with Dermal fillers. Hands are often the area of the body that gives away the age of the patient. They often don’t match the level of care or maintenance we dedicate to the face and often are responsible for giving away the real age of a person. In these before and after photographs, the hands of this lovely and elegant patient have been treated with Radiesse, although I normally utilise Juvederm Voluma. I definitely suggest the use a cannula with only 2 entry points to minimise the post operative trauma and of course the patient’s discomfort during the procedure.

I think it’s very important to see a particular feature like the lips (in this case), or a chin, or the cheeks, as part of a whole given by the rest of the face. We need to shape the lips to fit the rest, or to enhance it. There must always be a natural flow of the different components that will strengthen the image of the whole as a natural beauty. Hopefully more and more injectors are moving away from just treating only one part of the whole picture, and are also starting to utilise more advanced techniques such as myomodulation and non surgical lip lift () to create more natural and longer lasting results.

Facial reshaping. This is an interesting case in which I gave more balance to the overall appearance of the face by working on the nose (non-surgical rhinoplasty), and bringing the chin forward. You can appreciate straight away the more armonious and balanced appearance of the final result. We need to remember to always look at a person as a whole, and not as the single components that are part of it.
