✨What Should Be in a Care Plan Review?✨

Under the Aged Care Act and the Aged Care Quality Standards, regular care plan reviews are not optional—they’re essential to delivering safe, effective, and person-centred care.

Here’s what MUST be included in a care plan review for a Home Care Package client:

✅ Client involvement – The client (and/or their representative) must be actively involved in the review process.
✅ Current needs and goals – Are their health, mobility, cognitive, and social needs still the same? Have their goals changed?
✅ Risks and safety – Review of falls risk, medication safety, home environment, and emergency planning.
✅ Services being delivered – Are the current services meeting the client’s needs and preferences? Any gaps?
✅ Budget and funding – Review spending, unspent funds, and alignment with care goals.
✅ Cultural, spiritual, and emotional needs – These must be recognised and respected.
✅ Changes in health or circumstances – Hospital visits, new diagnoses, or carer stress? These must be factored in.
✅ Documentation – All discussions and decisions must be clearly documented and shared with the client.

💡 Care plans should be reviewed at least every 12 months, or sooner if the client’s needs change.

🛡️ This supports Quality Standards 1, 2, 3 & 7, ensuring dignity, choice, and clinical safety.

The Self-Assessment as we know was to be replaced by two new pre-audit assessment tools

The AECT and CDECT tools (as of 20th May 2025) replace the draft pre-audit preparation tool and request for information templates.

Who, what, when, how?

Let’s break it down:

AECT = (Draft) Audit Evidence Collection Tool

There are 3 versions.

1. Registration – Registering for the first time
2. Renewal – Renew your Registration - Same as the new provider option with columns for service specific examples
3. Variation – If you seek to add categories 4, 5 or 6 to your registration

Applicants must complete the Audit Evidence Collection Tool (AECT) and submit supporting documents to the commission as part of the audit process.

The commission will use the AECT to collect evidence about an applicant’s systems and processes, and how they deliver high-quality care to older people.

CDECT = (Draft) Care Delivery Evidence Collection Tool

Providers renewing their registration must also complete the Care Delivery Evidence Collection Tool (CDECT).

The commission use the CDECT to collect evidence about how a provider’s systems and processes are embedded at a service.

There are two versions of the CDECT, depending on the service type:
• Care Delivery Evidence Collection Tool – Care delivery location (Category 4 & 5)
• Care Delivery Evidence Collection Tool – Residential (Category 6)
