Models of Pregnancy Care

🤰Congratulations you are pregnant, what are your options for pregnancy care?

💜Midwifery Led Continuity of care (Midwifery Group Practice)

Your care is provided by a midwife or group of midwives that you will meet during your antenatal appointments, this model is provided in most public hospitals. Midwives in this model can also provide care for women with complicated pregnancies by working collaboratively with doctors and other health professionals. This model provides benefits to mother and baby and is based on the philosophy of women centred care and normal physiological birth.

💜GP share care

Your care is shared between your own GP and the local hospital’s midwives and doctors. This can provide easier access for appointments and familiarity with your own GP. Not all GPs provide maternity care.

💜Private Obstetric Care

Your care is provided by an Obstetrician who is a doctor who specialised in caring for women with complicated pregnancies or special circumstances. This will mean you will often birth at a private hospital.

💜Public Hospital Maternity Care

Antenatal care will be provided by hospital outpatient clinics by hospital midwives and/or doctors. Care could also include a multi-disciplinary team such as obstetric medical doctors, diabetic educators or neonatologists if needed. Collaborative intrapartum(labour/birth) and postpartum care is provided by the hospital midwives and doctors.

💜Private Midwife Care

Your care is provided by a midwife or group of midwives in a private practice, for your pregnancy, birth and postnatal care. You have a choice to birth in hospital or a home.

There are more varieties of shared care, please see what is offered in your area. Most models of care require a GP referral and remember to get in quick as some models such as MGP book out quickly due to number restraints.

Upon leaving the hospital, it is common for women to receive routine discharge information. However, there are instances where they are not provided with consistent guidance on how to monitor their own health and recognise warning signs of complications.⁠

The postpartum experience can differ significantly among women and even between different babies, making it challenging to determine what is normal and what is not. ⁠

I find great fulfillment in assisting and providing advice to parents and newly formed families within this particular field. Within my role, I have provided aid to a mother who recently gave birth to a baby with a complicated medical condition. I have also offered support to a pregnant woman who is expecting her second child after going through a challenging battle with cancer with her first child. Furthermore, I have extended my support to a first-time mother as she embarks on the journey of introducing new and diverse foods to her baby. Lastly, I provided assistance to a friend who is managing her baby's eczema.⁠

Transitioning into parenthood is a significant and transformative experience in an individual's life. Amidst the challenges of sleep deprivation, feeding, and diaper changes, it is crucial to prioritise one's mental well-being and engage in self-care practices. The postpartum period can affect mental health in various ways due to hormonal fluctuations, sleep deprivation, the birthing process, and available support. Following childbirth, it is common to experience "baby blues" for up to two weeks, characterised by emotional sensitivity, tearfulness, and anxiety. In most cases, receiving support and understanding from loved ones can alleviate these feelings. However, if these symptoms persist, worsen, or raise concerns, it is essential to seek professional help, as they may indicate postpartum depression or even psychosis.⁠

Fortunately, there are numerous resources available to assist new parents in navigating this transitional period. Seeking guidance from trusted healthcare professionals like general practitioners, obstetricians, or midwives, as well as contacting helplines such as PANDA, Beyond Blue, or Red Nose (for child loss), can provide valuable support. ⁠

Ensuring self-care is a priority during the postpartum phase is vital for maintaining the mental health of new mothers. Remember to grant yourself love, rest, and moments of pure joy. 🌜 Baby's Sleep Patterns . . . What is considered normal and not so normal? 🌛⁣
Understanding your little one's sleep patterns can be a crazy and beautiful journey full of highs and not so highs! Mother’s Group conversations can often lead here with everyone talking about what their baby is doing and without meaning to you’re quickly comparing your baby’s sleep patterns to another baby.

It is absolutely ok to do this as long as you are mindful and aware that every baby is so different and you are the only one that understands your baby best. It does however help to understand what stage they are up to and have some guidance along the way . . .

Here's a snapshot at what's ‘normal’ and when to keep an eye on things:

0-2 Months:
✨ Newborns sleep 10-19 hours, with no real defined routine initially.
✨ Night and day blur initially; starts stabilising at around 2-4 months.
✨ Active during sleep with movements, grimacing, and sounds —completely normal.

2-12 Months:
✨ Babies sleep 9-10 hours at night, with 3-4 hours of daytime naps.
✨ Variability in sleep among babies; nap numbers decrease as they grow.
✨ Illness, routine changes, and milestones may disrupt sleep at this age.

The 4-Month Regression:
✨ Around 4 months, baby sleep transitions from baby-like sleep to more adult-like sleep.
✨ Sleep cycles change, leading to more frequent wake-ups.
✨ Baby may need help falling back asleep; this phase is common but can be challenging.

At 6 months:
✨ Many babies can (I say this so gently as I understand this can be triggering for Mama’s) sleep through the night, but 25-50% may still wake.
✨ Babies may wake frequently; self-soothers can drift back to sleep, others need more support.
✨ Tracking sleep patterns can provide insights; please seek advice if concerns arise.

Understanding your baby's sleep patterns can help you to navigate their patterns and to seek more support where needed.

* Vitamin D3: Reducing postpartum risks while boosting immunity and bone health for baby.
* Omega 3: Supporting mental health and aiding in baby's visual and neurological development.
* Probiotics: Supporting your digestion and fortifying the immune system for both you and baby.
* Calcium: Vital for bone strength, heart health, and clotting in early postpartum.
* Choline: Aiding in liver health for you and crucial for baby's brain and nervous system development.
* Iodine: Supporting your organ function and preventing developmental issues for baby.
* Iron: Essential for emotional stability and preventing postpartum complications for both you and baby.

My Journey into Motherhood: The real & honest version.
Becoming a Mum at 22 was definitely a scary and overwhelming time in my life, marked by a traumatic birth experience that left me feeling absolutely powerless. The medical decisions seemed completely out of my control, having to process this was really difficult and took some time.

Our challenges continued — mistakenly discharged after 24 hours and an emergency rush-back due to my sons Bilirubin (bil-ih-ROO-bin) is a yellowish pigment that is made during the breakdown of red blood cells) being so high. My precious baby boy was so lethargic, he wouldn't feed and he stopped breathing as we arrived at the nursery, literally every Mother’s worst nightmare. This was followed by two weeks in the Special Care Nursery. Jaundice treatments, infections, constant expressing, and the emotional & intense attempts at breastfeeding became my normal routine.

At home, a strict 3-hourly feeding regimen consumed my days: breastfeeding, bottle top-ups, expressing. Each day came with the pressure of a weight gain target to gain 30g a day, adding to the mental load and exhaustion.

Physically and emotionally healing from a traumatic vacuum & forceps delivery with an episiotomy was not easy. Unfortunately, my partner at the time provided minimal support during the hospital stay and when we arrived home. I was isolated and alone.
Breastfeeding with a ni**le shield, sleepy feedings, and constant worries about his health were extremely exhausting. Days seemed liked weeks, and isolation & paranoia started creeping in.

Thankfully I got through this scary time in my life but I now understand what it is like to feel like this and this is why I do what I do!
