If your upper traps feel tight all the time… and you’re moving carefully just to avoid setting them off, let’s clear something up.

Most ā€œtight trapsā€ aren’t tight.
They’re tired because they’ve been working overtime.
Sometimes the pain isn’t even coming from the traps — it can be referred from your neck. Your brain isn’t great at telling the difference.
In more persistent cases, we also look at brainstem sensitisation — especially if stress, jaw tension or upper neck stiffness are part of the picture.

And quite often? It’s a completely different muscle driving the discomfort.

If stretching hasn’t solved it, that’s a clue.

When muscles feel tight all the time, it’s usually because they’re working too hard for the strength they currently have. šŸ’Ŗ

Your upper traps often end up compensating when other neck and shoulder muscles aren’t doing their job.

And quite often, what feels like trap pain might be actually referred from the joints higher up in your neck.
🧠 The brain can misinterpret where the signal is coming from.

In persistent cases — especially when stress and jaw tension are involved — we also consider brainstem sensitisation.
If all the poking and prodding hasn’t helped, that’s your clue. šŸ”Ž

You may be treating the wrong structure, but a proper assessment changes everything.

These muscles don’t show clearly on scans, they mimic disc pain or sciatica, usyally missed in generic rehab and keep your pain cycling and are the 4 biggest offenders we see every week:

Your back pain might not be from your back šŸ¤”
The real reason might be one of these 4 hidden prankster musclesšŸ‘‡šŸ»

1ļøāƒ£The Psoas
The Original Hidden Prankster. This muscle attaches into the front of the spine, crosses your centre of gravity, and attaches into each hip at the front. It causes back pain on it’s own, and scores of different biomechanical issues. In our experience, it’s the biggest muscle that’s been missed when a patient has ā€œseen everyone and nothing is betterā€.
2ļøāƒ£ The Latissimus Dorsi
This muscle attaches from the back of the arm all the way down into the spine and pelvis via a fascial connection. It’s supplied by the nerves of the neck. So, if there is back and neck tightness on the same patient – this can be a muscle that sneaks up and causes issues (especially in patients with Whiplash).
3ļøāƒ£the Gluteus Minimus
This tiny muscle deep in your hip can cause pain which feels like sciatica. You know it’s this one because the leg pain is worse standing than sitting… good as start of day and worsens as day goes on… and no amount of stretching your back or piriformis has helped. Dry
needling is essential to release this as it’s a very deep muscle.
4ļøāƒ£The Scalenes
These muscles attach from your neck to the first rib, and can cause pain in the neck, shoulder blade and down the arm. They make your neck feel tight – but this muscle is key for ā€œstress-headsā€ because they can be used for breathing. Usually breathing should be
deep into the rib cage and lungs, but stress can cause your system to use the scalenes instead.

At The Headache and Pain Management Centre, we find that most lower back pain isn't just a muscle fluke—it's a lack of functional movement in 4 key areas. If one part of the chain is stuck, your spine pays the price.

Here is your Spinal Mobility Checklist:

1ļøāƒ£ Lumbar Range: Each segment of your spine (T12 to L5) should move like a smooth chain. If they move as one "block," you'll feel constant ache. ā›“ļø
2ļøāƒ£ Hip Range: If your hips "jam" when you bring your knee to your chest, they are overloading your back every time you move. šŸ”„

Think your neck pain is just "stress"? Think again. šŸ§ šŸ›‘

At The Headache and Pain Management Centre, we see many patients who are frustrated because their neck "tightness" never truly goes away.

The reason? Most treatments miss these 5 vital facts about neck pain relief and function:

1ļøāƒ£ The Great Divide: Your neck isn't just one unit. It functions in two sections: the Upper Neck (C1-C2) and the Lower Neck (C3 and below). An issue in one almost always creates a chain reaction in the other.
2ļøāƒ£ The Thoracic Link: You can't have full neck movement without your upper back. If your thoracic spine (between your shoulder blades) is locked, your neck can never turn fully.
3ļøāƒ£ Tight vs. Tired: Your brain often confuses muscle "tiredness" with "tightness". If you’re constantly rubbing your neck for only temporary relief, you likely need strength and endurance, not just another stretch.
4ļøāƒ£ It’s Probably Not Your Traps: That "bottom of the neck" ache is usually the Levator Scapulae. This muscle connects your shoulder blade to your upper neck, and surprisingly, back or arm weights can often be the fix.
5ļøāƒ£ The Brain’s Defense: Neck pain is usually physical, but your brain can trigger pain due to poor sleep, diet, or high stress levels as a way to "protect" you.
