Thank you to Peak Care Services and everyone who joined Thrive Together: Hospital to Home Transitions. The session highlighted the value of strong collaboration between hospitals, disability services, SDA and SIL providers, allied health professionals, support coordinators and families when supporting people to transition from hospital into long‑term housing.
It was encouraging to be part of conversations focused on practical approaches, partnership, and improving outcomes for people during complex transitions.

ADAPT Housing is hosting an on-site SDA Viewing Day, offering the opportunity to step through completed Specialist Disability Accommodation and see how different SDA design categories work in practice. This day is designed to support shared learning and informed planning for anyone interested in SDA and how it functions in real homes.
Attendees are welcome to explore the site at their own pace through self guided viewings, or join an optional walkthrough with an ADAPT Housing team member to discuss key design features and intent. The development includes a mix of High Physical Support and Robust SDA villas, thoughtfully designed and well separated across the site to support safety, suitability and long-term liveability.
