At Care by Phoenix our goal is to provide support and services to individuals with disabilities under the National Disability Insurance Scheme (NDIS) in Australia.
We aim to improve the quality of life for people with disabilities by offering a range of services tailored to their individual needs and goals. Key objectives of Care by Phoenix include:

A Support Coordinator helps you:
• Understand your funding
• Connect with providers
• Set up and manage supports
• Build skills to become more independent
In short: it helps you get the most out of your NDIS plan and stay in control of your supports.
At Care by Phoenix, our Support Coordination service helps you make the most of your National Disability Insurance Scheme (NDIS) plan with confidence.
We provide Level 2 Support Coordination, helping you:
• Understand your plan
• Connect with the right providers
• Manage and coordinate your supports effectively
Our team works closely with you to understand your goals and create a tailored support approach. We assist with organising services, coordinating providers, and adapting your plan as your needs change.
We offer ongoing, hands-on support to keep your NDIS plan running smoothly and aligned with your goals.

We are seeking compassionate and reliable Disability Support Workers to join our team to support clients in Caboolture

The right candidate will have the following:
• Disability Worker Screening Card (Mandatory)
• Working With Children Check (Mandatory)
• Drivers License (Mandatory)
• Your own vehicle - registered and fully insured (Mandatory)
• Current First Aid certificate (or willing to obtain)
• Current CPR training (or willing to obtain)
• Current Police Clearance less than 3 months old (or willing to obtain)
• Cert III in Individual Support or above (or willing to obtain)
• NDIS Worker Orientation Modules (or willing to obtain)
• Any other beneficial additional training will be highly regarded
Personal Care experience preferred but not essential.
Epilepsy knowledge, training and management preferred but not essential.
Evening / night shifts and possible sleepovers available.

Job Type: Casual
Licence/Certification:
• First Aid Certification (Preferred)
