I missed something obvious.

A psychologist pointed out that they send 5-6 questionnaires per client for ADHD and autism assessments. On ClientForms, that meant 5-6 separate links. Client enters their name, date of birth, and contact details every single time.

That's not frictionless. That's asking someone with ADHD to repeat the same admin task six times in a row.

Up to 85% of people with ADHD have working memory deficits. In 1956, psychologist George Miller published research suggesting working memory has limits, often cited as "7 plus or minus 2" items. Later research from Nelson Cowan refined this: when you can't group information into meaningful chunks, the limit is closer to 3-4 items.

Assessment questions are hard to chunk because each one asks something different. A long unbroken form creates exactly the conditions that lead to cognitive overload, disengagement, or impulsive answering.

Every form on clientforms.app is sectioned into groups of 5-7 questions with natural pause points. The DASS-21 becomes three sections of seven. The PCL-5 becomes four sections. They're designed around how memory actually works.

150 psychology assessment forms. That felt like a milestone worth noting.

I started with three: the DSM-5 ADHD assessments for adults, children, and intellectual disability. Those first forms taught me what I was actually building. Not just digital questionnaires, but something designed around how ADHD and autistic brains process information.

Now there's 150 forms covering depression, anxiety, trauma, autism, personality, eating disorders, OCD, and more. Every single one built with time estimates for time blindness, grouped questions to reduce cognitive load, progress indicators that actually help, and calmer colours throughout. No patient login required.

Aiming for 180 next.

But the bigger focus right now is Cliniko integration. I want assessment results to sync automatically as treatment notes. Patient completes a form, results just appear in Cliniko. No copy-paste, no extra login, no friction. Assessments that disappear into the workflow you already have.

I've been building in silence. That's about to change.

For people with ADHD, perfectionism isn't about high standards. It's protection.

Rejection Sensitive Dysphoria is real. It's not being "fragile" or "weak". It's how the ADHD brain processes emotional and social cues. Research shows that for adults with ADHD, even mild criticism can trigger persistent feelings of failure.

So you wait. You tell yourself it needs to be "ready" first. Really, it's protection.

I built ClientForms to reduce anxiety for neurodivergent people. Yet I've been working in silence because of the same fears.

It’s a digital assessment tool for psychologists working with ADHD and autistic clients. Designed to replace paper forms and clunky PDFs with something genuinely usable.

Most online forms aren't designed for neurodivergent brains. For someone with time blindness, not knowing how long a form takes is genuinely stressful. Scattered questions, cluttered layouts, no sense of progress. Small things that add up.

So, I built something different.
